Job summary
Aveley, South Ockendon and Purfleet Primary Care Network (ASOP PCN) has a vacancy for a UK registered nurse, experienced in long term conditions (cardiovascular disease, hypertension, diabetes and stroke prevention, as well as understanding mental health and wellbeing). This role will be across the whole PCN, a collaboration of 6 GP practices, although the base will, initially, be Aveley Community Hub. A degree of flexibility will be required as this base may change in the future. There will also be an expectation of some home-working each week.
Main duties of the job
This role will support people to take pro-active steps to improve their physical and mental health conditions, based on what matters to them, and their health risk. The nurse will support people to develop their self-care knowledge, skills, confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health-related behaviours. The nurse will coach and motivate people through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support. They will also manage treatment plans.
About us
ASOP PCN consists of 6 different surgeries based around Aveley, South Ockendon and Purfleet Area. The PCN population is ~40K patients. We encourage the staff to work across all surgeries and try and integrate with all practices as much as possible. We use SystmOne as our clinical system.
Primary care networks (PCNs) offer a way of organising care, bringing together all local health and care services in a local area to deliver more joined up care to local populations (covering around 40,000 people in ASOP). When fully developed, PCNs will expand into integrated neighbourhood teams which will include: GP services, pharmacies, community health services, mental health services, adult social care and other council services and voluntary organisations.
