Pharmacy Technician

Job summary

In partnership with Canvey Island Primary Care Network, GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is hosting an innovative service to recruit a dedicated part-time pharmacy technician to the network on a part time basis, working 22.5 hours per week to ideally start in February 2024.The successful candidate will work across all six surgeries within our primary care network (PCN):

• Oaklands Surgery
• Third Avenue Doctors' Surgery
• The Island Surgery
• The Surgery - Hawkesbury Road
• The Community Practice
• Canvey Village Surgery

Main duties of the job

• Work with the clinical pharmacists & clinicians in accordance with agreed practice prescribing standard operating procedures.
• Process repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines. Processing prescriptions in line with agreed protocols & within your competency.
• As determined by the PCN, support medication reviews/monitoring & medicines reconciliation post discharge (or other transfer of care) & synchronising medicines for patients, especially if transferring between care settings, linking with local community pharmacists.
• Undertake patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
• Support clinicians and clinical pharmacists in structured medication reviews (SMR).
• Manage shared care protocols and liaise with clinical pharmacist for more complex patients.
• Develop and maintain medicines related communication to all practice staff.
• Assist with the development and review of medicines audits.
• Provide contact for patients and colleagues with medication queries.
• Support with project work/prescribing initiatives/clinical audits under the support of the pharmacist.
• Liaise with clinicians and staff to resolve prescribing queries.
• Support the management and development of the prescribing systems to ensure safe, effective, and quality prescribing within member practices in the network.

GP Healthcare Alliance is a company, originally formed as a GP federation, made up of 18 practices who work together to provide healthcare for our practices and patients, in Castle Point, Rochford and Rayleigh, Essex. All practice members remain independent contractors, but work collaboratively to improve healthcare services within our area.
