Job summary
We have an exciting, new opportunity for a care co-ordinator to join The Dengie and South Woodham Ferrers Primary Care Network, to support our GP practices across the local area and to assist in re-shaping primary care across our communities on a full-time basis.
Main duties of the job
The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.

The Dengie and South Woodham Ferrers Primary Care Network is a collaboration of seven GP practices serving approximately 45,000 patients from the Dengie, Bicknacre, Burnham-on-Crouch and South Woodham Ferrers area. Person Specification
Knowledge, Skills and Experience:
Essential
- Excellent person-centred planning experience;
- Strong local knowledge for accessing key support;
- Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
- Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification;
- Strong listening and communication skills;
- Familiar with local resources and services, including how to access them;
- Motivated by helping people, with care and empathy;
- Able to deliver person-centred support in a non-judgmental way;
- Experience of completing holistic person-centred care planning assessments.
