Job summary

Canvey Island Primary Care Network is seeking to appoint an enthusiastic and committed nurse practitioner (NP) to join our PCNon afull-time basis, for 37.5 hours per week,to work across all six practices within our network. There is flexibility to discuss hours at interview.

The six practices within our PCN are: Oaklands Surgery, Third Avenue Doctors Surgery, The Island Surgery, The Surgery - Hawkesbury Road, The Community Practice and Canvey Village Surgery.

Main duties of the job

The main aim of this role is to increase health and wellbeing of patients through improved screening, identification, and disease management. This will entail a mixture of clinics and home visits, as directed.For further information about this role, please see the attached job description.

About us

Canvey Island PCN is a network made up of 6 surgeries who have 42,000 registered patients. Our vision is to work in partnership with our colleagues and patients to provide the best primary care services in the community.

Job responsibilities

  • Assess, aid diagnosis, plan, implement and evaluate treatment/interventions and care for patients;
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly;
  • Assess diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients with or at risk of developing a long-term condition;
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan;
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice;
  • Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions;
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care;
  • Support and educate patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care;
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
