Job summary
Rayleigh and District Primary Care Network (PCN) have an exciting opportunity to join our PCN to help with long term condition disease management for our local population. The base of the role will be at the PCN offices in Hockley, but the successful candidate will need to have the ability to undertaken home visits and access venues across the patch.

About us
Rayleigh and District Primary Care Network (PCN) is made up of six GP surgeries, covering a population of approximately 56,000 from the Rayleigh, Hullbridge and Hockley areas. Our PCN serves an area with a diverse population, providing a varied case mix. Our vision is to work in partnership with our colleagues and patients to provide the best primary care services in the community.

The role will involve providing outreach sessions, offering health promotion and assessments in the local community centres, sheltered houses, and in the patients' own homes. The post-holder will work alongside a healthcare assistant and care coordinator, to increase health and wellbeing of patients through improved screening, identification, and disease management. They will work collaboratively with our clinical team and member practices on Quality Outcomes Framework (QOF) measures which can include:
- Cardiovascular disease management
- Diabetes
- Learning disability
- Obesity
- Cancer screening and early identification
- COPD/Asthma
- Flu / COVID and immunisations

- A relevant nursing degree
- Current NMC registration with an active PIN
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications
Knowledge, Skills and Experience:
Essential
- Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
- Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
- Knowledge, and experience of its application, in supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.
