Job summary
Taunton Deane West Primary Care Network (PCN) is seeking a highly motivated and experienced PCN Manager to lead our operational and strategic development. This exciting role combines operations management with digital and transformation leadership, offering the opportunity to shape the future of primary care delivery across our network. The post holder will work closely with the Clinical Director, PCN Board and member practices to deliver the requirements of the Network Contract Directed Enhanced Services (DES) and local priorities. They will provide line management to PCN staff, oversee service delivery, contracts, finances, and act as the first point of contact for practices and commissioners. As Digital and Transformation Lead, the post holder will drive service improvement, implement innovative digital solutions, and support sustainable change aligned with Integrated Care System (ICS) priorities. This is a unique role for an individual with proven experience in delivering change and managing services who is passionate about improving patient care.
This is a full-time role, however applicants looking for part-time also considered.

Main duties of the job
The PCN Manager will provide both operational and strategic management across the network. Key responsibilities include leading the implementation of the Network Contract DES, managing contracts and finances, and overseeing recruitment, supervision, and development of PCN-employed staff. The post holder will coordinate PCN meetings, manage subcontracted services and act as a key liaison with commissioners, neighbouring PCNs and local partners. As Digital and Transformation Lead, they will identify opportunities for innovation, support practices to adopt new technologies, and lead service redesign projects to improve patient and staff experience. The role requires data-driven decision-making, using insight and performance information to inform strategy and monitor impact. The post holder will also represent the PCN at system-level forums, ensuring local priorities are aligned with broader ICS objectives, and supporting collaborative, sustainable improvements in patient care.

Main Duties
- Take the lead (along with the PCN Clinical Director and PCN Board) to oversee the management and delivery of the PCN DES requirements.

The core objective of the PCN Digital and Transformation Lead role is to support the PCN and its practices to deliver ongoing improvement to services. The role will require experience in business / service improvement with expertise in transformational change and a good understanding of general practice systems and processes, including broad familiarity with general practice digital systems and how they can be used to deliver improvement. This expertise will be used to:
- Work alongside practices to understand current challenges to effective service delivery and identify solutions, including existing best practice within the PCN.
- Drive the planning and prioritisation of changes that will improve delivery of care and improve patient and staff experience in the practices within the PCN.
- Ensure all change is ultimately focussed on improving services for patients.
- Build relationships across the PCN to develop consensus for improvements and to ensure business and clinical staff are fully engaged throughout the process of selection, implementation and deployment of change.
- Develop and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering the potential impacts on the wider organisation, and determining resource requirements.
