The Proactive Care Team (PAC Team) visits patients within their own homes and provides them with comprehensive assessments to enable patients to self-manage and feel confident with their long-term health conditions. Proactive Care is a national initiative which enables patients to self-manage their health. In TDW the Proactive Care team currently has five existing team members, a Clinical Director, a nurse, two occupational therapists, and a data IT co-ordinator. The team is looking to welcome an additional team member to provide enhanced administrative support for the day-to-day running of the service. The specific aims of proactive care are to improve health outcomes and patient experience by delaying the onset of health deterioration where possible, maintaining independent living, and reducing avoidable exacerbations of ill health, thereby reducing the use of unplanned care.

Main duties of the job

  • Book home visits for the PAC team
  • Manage the diaries of multiple members of the team
  • Oversee the PAC Team inbox on Outlook
  • Oversee the answerphone messages
  • Undertake buddying requirements for PAC team members undertaking home visits (as per the Lone Working Policy)
  • Arrange review appointments for patients
  • Gather patient feedback about the service
  • Keep up to date with available resources including neighbourhood services
  • Signpost patients to resources for self-management
  • Encourage patients to engage with the service
  • Engage with stakeholders and maintain a good relationship
  • Attend meetings related to the Proactive Care Team when required
