Main duties of the job

The Role

As a Band 5 Assistant Commissioning Nurse in NHS Continuing Healthcare, you'll play a key role in reviewing eligibility for the NHS Continuing Healthcare and NHS Funded Nursing Care in a variety of settings including patients own home, care home or other care facility.

You'll help ensure that people receive the right support, whether through NHS Continuing Healthcare (CHC), Funded Nursing Care, or jointly funded care packages.

You'll work closely with patients, their families / representatives, and health and social care professionals to ensure that care for eligible people is delivered safely and effectively. Your expertise will help shape care plans, influence quality of care, and safeguard those who are most vulnerable.

What You'll Be Doing

Triaging new referrals to the service

Reviewing eligibility for NHS Continuing Healthcare and Funded Nursing Care in a range of settings (homes, care facilities, etc.);

Supporting patients, carers, and professionals with expert advice and guidance;

Ensuring that care plans meet the needs of patients in a safe and person-centred way;

Monitoring and improving the quality of care provided;

Working alongside local healthcare teams, NHS providers, and social care colleagues to ensure the best possible outcomes for our communities.

Key responsibilities

This will be varied and will adapt over time as both the ICB/S develops. However, thepostholder is expected to fulfil the following key functions:

  • Review eligibility for the NHS Continuing Healthcare and NHS Funded Nursing
  • Care in a variety of settings including patients own home, care home or other carefacility.
  • Review the care planning, to ensure that all care needs are being met safely andeffectively.
  • Monitor and influence the quality of care that is provided to patients; drawing onprevious skills and knowledge to improve the patient experience of care.
  • Provide professional advice to patients, carers and multi-agency colleagues,including signposting to appropriate services and resources.
  • Triage new referrals as part of duty desk cover.

Key working relationships

The post holder will have regular contact with a wide range of staff at all levels within theICB/S, with senior and other representatives of local stakeholders, as well as withpatients and their representatives. Some of the key direct relationships include:

  • Cheshire and Merseyside ICB/S executive teams
  • Cheshire and Merseyside ICB leadership teams
  • Chief Information Officers / Digital Leads
  • Clinical Leaders
  • Colleagues within NHSE/I

Main duties and responsibilities

  • First point of contact for referrals to service and provide duty nurse cover onallocated days
  • Understanding of PHBs
  • Co-ordinating assessment for NHS Funded Nursing Care and NHS Continuing
  • Healthcare as close to the service user as feasible
  • To provide clinical commissioning oversight to applications and assessmentsconcerning joint funded packages of care
  • Seek authorisation of eligibility recommendations via Line Manager
  • Case management as allocated by the Line Manager
  • On-going review of service users eligible for CHC, NHS funded care inaccordance with National Framework for NHS Funded Care, and Joint fundedcare packages of care
  • Undertake urgent reviews of service users, where quality and safety concernshave been identified and report in a timely manner
  • To prepare and complete comprehensive documentation/tools to enable thelevel of NHS funding to be determined within the parameters of the NationalFramework for Continuing Healthcare and Funded Nursing Care and Joint
  • Funding arrangements locally
  • Accurately input information onto departmental databases and tools used inContinuing Healthcare adhering to NMC standards for record keeping
  • To quality assure external recommendations for CHC/FNC
  • Completing Continuing Healthcare Checklists, where appropriate, and thatthese are processed following service operating procedures
  • To provide assurance with regard to quality of care of individual packages byundertaking reviews as agreed
  • To support the delivery of local discharge to assess processes
  • To actively promote communication and partnership working between thestatutory, independent and voluntary sectors
  • To communicate relevant information and to provide advice and support onfunded nursing/continuing health care to service users and carers
  • To liaise with other health professionals on clinical matters as part of theassessment process
  • To maintain professional confidentiality in all matters relating to patient care
  • To participate in informal and formal education and training as requested
