MIAA Solutions is advertising the above role on behalf of Neston and Willaston PCN, who are seeking to recruit a PCN Diabetic Specialist Nurse, to join the growing PCN team.
Salary: Band 8
Full time 37.5 hours
Location:The Neston and Willaston PCN consists of Neston Surgery, Neston Medical Centre and Willaston Surgery.

Main duties of the job
The post holder will be an expert in Diabetes and will offer specialist diabetes management for the practices within Neston and Willaston PCN.
The role will involve insulin initiation, commencing GLP1s and have an awareness of emerging treatments. The individual will maintain their knowledge and skills and be able to support practice staff including, Practice nurses, ANPs and GPs to develop their knowledge and skills around the management of diabetes.

Neston and Willaston PCN is made up of three surgeries,
Neston Surgery, Neston Medical Centre and Willaston Surgery and has a
population of 21,000 patients. Our practices work closely together and with the
local community with the aim of creating a healthier future for our patient
population. Within the PCN there are many established roles including visiting
Paramedics, Clinical Pharmacy Team, Mental Health Practitioner, Social
Prescribing Link Worker, Wellbeing Co Ordinator, Counselling Service and a
Physio Service.

- To work with clinical teams to promote excellent standards of care to people with diabetes inline with evidence-based practice.
- To demonstrate advanced knowledge of diabetes and its co-morbidities using appropriate skills and innovative practice to ensure the delivery of safe care that is up to date, evidence based and within local and national guidelines.
- To screen, prevent and detect early complications of diabetes and its co-morbidities whether physical, psychological or social. To maintain links with the diabetes specialist services in secondary care, referring patients to specialist clinics
- To work autonomously as an independent practitioner, to prescribe demonstrating expertise in the assessment, implementation, planning and evaluation of patient care
- In collaboration with the practices facilitate the continual development of diabetes services in order to develop and maintain an innovative service
- To expand and develop the role to meet the changing needs of patients

- Excellent interpersonal, influencing and negotiating skills
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions
- Experience of policies and procedures
- Good IT Skills
- Able to obtain and analyse complex technical information
