As part of HEEs Primary Care Educational Roadmap to Practice, Widnes Highfield Health are looking for a Paramedic to work across3 GP Practices in Widnes

Please note this role is only open to applicants within the UK.

Main duties of the job

The role will be to provide high-quality cost-effective care service to patients in both Practices; undertaking clinical assessment, diagnosis, and evaluation of care while working within defined procedures and protocols set by the Medical manager/Medical Director. To be responsible for seeing patients at the practices and visiting patients in their place of residence, who present with a wide range of minor illnesses, urgent and same day requests for treatment and chronic disease management, providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. Applicants must have 5-year post-qualification experience.

Key Responsibilities

  • Co-develop and provide an alternative model to urgent and same day GP home visit for the network
  • Co-develop and provide a planned long-term condition home visiting service
  • Work as part of an MDT within the PCN; Consisting of Doctors, Pharmacist, Care Coordinators, Social Prescribers LW, Community figures and more
  • Triage and treat patients as appropriate, including same day triage. Making any necessary referrals to other members of the primary health care team or organise admission if required
  • Provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the Federations social prescribing service, and where appropriate, other community or voluntary services

Be able to:

  1. Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance
  2. Perform and interpret ECGs
  3. Perform investigatory procedures as required; and
  4. Undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance
  • Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)
  • Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered
  • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required
  • Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice; and
  • Undertake work in line with PCN directed priorities such as;

  1. Support national screening programmes
  2. Support immunization programmes
  • Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service
  • Carry out Avoiding Unplanned Admission reviews
  • Liaising with at home teams and providing post discharge care
  • Provide remote monitoring, when possible, for patient's in-between hospital visits (Opportunity to develop and innovate how this looks like)
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations
  • Be able to work from the member practices and within communities as an autonomous practitioner caring for patients and families
  • To impart knowledge and skills formally and informally to colleagues promoting peer review and best practice within the work environment
  • Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care

Pathological specimens and investigatory procedures

  • Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs
  • Administration and professional responsibilities
  • Participate in the administrative and professional responsibilities of the practice team
  • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
  • Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
  • Ensure appropriate coding for items of service claims are made accurately, reporting any problems to the practice administrator or Operations Manager
  • Ensure accurate completion of all necessary documentation associated with patient health care
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
  • Attend and participate in PCN and practice meetings as required
  • Restocking and maintenance of clinical area, visiting bags as required

Training and personal development

  • Training requirements will be provided and monitored according to HEEs The First Contact Practitioners and Advanced Practitioners in Primary Care: (Paramedic) A Roadmap to Practice guide

  • As per the roadmap to practice, training requirements will commence for stage 2 following completion of stage 1, stage 2 must be completed in practice within the specified time period
  • Training requirements will be monitored by yearly appraisal and will be in accordance with PCN requirements, including taking responsibility for maintaining a record of own personal and/or professional development
  • Personal development will be encouraged and supported by the Federation. It is the individuals responsibility to remain up to date with recent developments
  • Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring mandatory requirements are met
  • If it is necessary to expand the role to include additional responsibilities, full training will be given
  • Develop and maintain a Personal Learning Plan

Liaison

  • As well as the GP and nursing team, there is a need to work closely with admin teams of the member practice you are in to ensure the smooth running of the practice, reporting any problems encountered to the relevant person or to the Operations Manager
  • Pivotal you establish and maintain good liaison with all member practices, community services and agencies, including secondary care
  • Play an important role in building strong relationships for the PCN with external services you come in contact; raising awareness of the PCN values and initiatives as and when appropriate

Meetings

  • It will be necessary to attend and contribute to various PCN meetings as requested. The only reason for not attending will be annual, study or sick leave
