MIAA Solutions is advertising the above role on behalf of Healthier South Wirral Primary Care Network, who are seeking to recruit an Advanced Nurse Practitioner on a permanent basis. Full time preferred but part time considered.
Salary: dependant upon experience.
Location: Bebington, Wirral, CH63 7RX

Main duties of the job
To work as an autonomous practitioner, responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Job responsibilities
The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Develop, implement and embed health promotion and well-being programmes
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Undertake the collection of pathological specimens
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure read codes are used effectively
- Maintain chronic disease registers
- Support with care home ward rounds
- Chaperone patients where necessary
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Recognise, assess and refer patients presenting with mental health needs
- Implement vaccination programmes for adults and children
- Extended and supplementary prescriber, adhering to extant guidance

- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
