Job summary
The successful candidate will be
based at Wigan Central comprising of 9 GP surgeries. We are looking to recruit
a new Care Coordinator to join our extensive practice and PCN teams. The job
requires a caring, dedicated, reliable, person-focused candidate who enjoys
working with a wide range of people. They will have good organisational and
time management skills, have data collection abilities and be able to work on
new projects. They will be highly motivated and proactive with a flexible
attitude, keen to work and learn as part of a team and committed to providing
people, their families and carers with high quality support. They would be
responsible for overseeing the social media pages and highlighting positive
achievements/outcomes from the PCN and sharing local events.
Main duties of the job
Work with people, their families & carers, to improve
people's understanding of their condition. Offer support to develop &
review personalised care & support plans to manage their needs &
achieve better healthcare outcomes.
Provide co-ordination & navigation for people &
their carers across health & care services. Helping to ensure patients
receive a joined-up service & the appropriate support from the right person
at the right time.
Collecting data to provide information to enable the PCN to
make decisions about projects to pursue for DES and IIF targets.
Using digital platforms to collect and share information to
find any gaps in care navigations between various teams and services such as
practices, PCNs, community providers and other care providers in the wider
system.
Work collaboratively with GPs & other primary care
professionals within the PCN team including GP Assistants, Nurse Practitioners,
Nurses, Pharmacists, Pharmacy Technicians, Physiotherapists, Mental Health
Practitioners, Community Link Workers and Paramedics. Work with this broad team
to identify & manage caseload, which may include patients with long-term
health conditions & where appropriate, refer to other health professionals
within the PCN.
Support the co-ordination & delivery of
multidisciplinary teams with the PCN.
Raise awareness of how to identify patients who may benefit
from shared decision making & support PCN staff & people to be more
prepared to have shared decision-making conversations.

- Use EMIS to carry out searches and audits; and develop templates and protocols to support the PCN and practices
- Maintain accurate data within clinical systems.
- Run system reports and extract data as required by the PCN Senior Leadership Team for activity reporting and audits, using EMIS and other data sources
- Identify and correct coding issues to ensure accurate records.
- Support updates to templates, protocols and data quality tools.

2. Financial Incentive Schemes (QOF, LES/LIS, IIF, PCN DES, GPQC)
- Provide admin support for incentive schemes.
- Monitor indicators, highlight gaps and ensure accurate data capture.
- Assist with claims and performance reporting.
- Work with clinicians to improve achievement and maintain funding related data.
3. Recalls & Patient Communication
- Coordinate recalls for LTCs, vaccinations, screening and reviews.
- Contact patients via phone, letter, SMS or digital systems.
- Support booking of review appointments and respond to recall queries.
- Track recall progress and update internal logs/dashboards.
4. Campaigns & Patient Engagement
- Support patient outreach and health promotion campaigns.
- Prepare data lists, messages and admin materials.
5. General Administration
- Maintain confidentiality and follow IG requirements.
- Summarise new patient records and support registrations/deductions.
- Provide wider admin support as required.
- Carry out additional tasks to support practice operations.
6. PCN Systems Support
- Support Wigan Central PCN and its member practices by promoting and developing the use of clinical systems and digital products.
- Support the use of digital tools such as Ardens, Accurx, and the NHS App.
- Support social media screen content management.
- Highlight any anomalies in information, ideas for new ways of working and details of any potential blockers to tasks.
