Job summary

Wigan Borough Federated Healthcare on behalf of Primary Care Networks are seeking to recruit band 7 Clinical Pharmacists to join their existing friendly team to further improve the already excellent quality of service offered to the local population

The Clinical Pharmacists will have a key role in supporting delivery of the new Network Contract DES Service specifications.

We see the role of the Pharmacists as critical to realising our ambitions to improve patient outcomes.

The role will focus on PCN initiatives as set out in the PCN contract, NHS Long Term Plan and deliverables agreed by the network members to improve place based patient care. Key responsibilities of the role may include:

The Clinical Pharmacist is very much a patient-facing role, with plenty of opportunity to utilise and develop clinical skills to improve the health outcomes for the local community. This will long term condition management, supporting transfer of care prescribing, offering advice to polypharmacy patients and to support the GP practices to develop a clinical pharmacist role. As a PCN Clinical Pharmacist you will be a primary point of contact to provide specialist advice relating to medication related queries and be able to signpost and liaise with other members of the multidisciplinary team.

Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs and tackling inequalities across the Primary Care Network geography.

Develop relationships and work closely with other pharmacy professionals across the wider health and social care economy.

Provide leadership on person centered medicines optimisation and quality improvement.

An exciting opportunity has arisen for a band 7 Clinical Pharmacist to work alongside our existing friendly & supportive team to develop a pioneering role within primary care. Wigan Federated Borough Healthcare is looking for a clinical pharmacist to join either for Wigan Central or Wigan North.

The role will focus on PCN initiatives as set out in the PCN contract, NHS Long Term Plan and deliverables agreed by the network members to improve place based patient care. Key responsibilities of the role may include:

The post holders will play an integral role within the network and will be part of the MDT supporting monthly neighbourhood meetings with other providers and stakeholders to improve patient pathways.

We are looking for adaptable and enthusiastic candidates, who are looking to use and develop excellent clinical skills. The successful candidate will be required to work independently at a network and practice level, but also as part of a MDT to deliver safe and effective use of medicines by applying their pharmaceutical skills in a variety of tasks as part of the medicines optimisation agenda in a timely manner.

Clinical Responsibilities

The post holders will:

Provide expertise in holistic and clinicial medication reviews and address public health and social needs of patients.

Be a key contact for and manage patients and health care professionals medicine queries in a professional and timely manner.

Contribute to reductions in medicine related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues.

Work within the practice teams to undertake medication reviews particularly in high risk groups.

Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

See patients with chronic medical problems where medicine optimisation is required.

Represent the PCNs in a positive way.

Communicate and work effectively with individuals in other departments/agencies to meet patient needs.

Be available to undertake a variety of duties including telephone consultations and queries, attending practice based MDT meetings and dealing with queries, paperwork and correspondence in a timely fashion.

Act as key contact and as a source of medicines information for all of the practices teams and patients.

  • Make professionally autonomous decisions but also be aware of when it is necessary to liaise with other members of the healthcare team for support.
  • Provide counselling and health education.
  • Prescribe in accordance with the local prescribing formulary or working to PGDs whenever this is clinically appropriate.
  • Refer to other care providers as deemed necessary
  • Comply with relevant Locality policy/guidelines, e.g. Child Protection, confidentially issues etc.

  • Minimum of 2 years post-qualification experience
