Job summary
The Practice Nurse plays a vital role in delivering high-quality nursing care within primary care settings across Barking & Dagenham. Working in line with NMC standards, the post holder provides autonomous and collaborative clinical care, supporting patients across a wide range of health needs while contributing to effective multidisciplinary working.

Main duties of the job
The Practice Nurse undertakes comprehensive patient assessments, health screening, chronic disease management, immunisations, and health promotion activities. The role includes diagnosing and managing acute and long-term conditions within scope of practice, delivering evidence-based care in line with clinical guidelines, maintaining accurate clinical records, and supporting quality improvement, audits, and the development of nursing protocols. The post holder also contributes to mentoring colleagues and supporting safe, effective service delivery.
