Job summary

We are seeking a motivated, innovative and experienced Clinical Pharmacist with proven leadership skills, in an autonomous position. You shall be accountable to the Board and GP Partners.

At this moment, we do not offer any sponsorships.

Main duties of the job

The post holder is a PCN Clinical Pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice and within the PCN. They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients.

Job description

Job responsibilities

  • Clinical Care & Patient Support
  • Assess and treat patients using expert knowledge of medicines.
  • Prescribe (or train to prescribe) and support the general practice team.
  • Manage care for patients with long-term conditions, complex polypharmacy, frailty, asthma, COPD, learning disabilities, or autism.
  • Conduct structured and new-patient medication reviews.
  • Provide proactive care for vulnerable or high-risk patients, helping prevent admissions/re-admissions.
  • Support patient/carer understanding, confidence, and adherence to medicines.
  • Signpost patients to the right healthcare professional.
  • Answer medication queries from patients and staff.
  • Medicines Optimisation & Prescribing
  • Lead on person-centred medicines optimisation, including antimicrobial stewardship.
  • Encourage safe, effective, and cost-efficient prescribing across the PCN.
  • Implement and embed robust repeat prescribing and medication monitoring systems.
  • Handle prescription queries and requests directly.
  • Supervise safe storage, rotation, and disposal of vaccines and drugs in line with infection control standards.
  • Promote self-care and reduce medicine waste.
  • Leadership, Collaboration & Integration
  • Provide leadership on prescribing systems, quality improvement, and service delivery.
  • Work as part of a multi-disciplinary team and integrate with wider healthcare services (community, hospital, mental health pharmacists).
  • Develop strong relationships across PCNs and health/social care networks.
  • Support and mentor junior pharmacists through structured supervision.
  • Contribute to service development and enhanced services delivery.

  • Improve prescribing quality through audits, NICE/CQC compliance, and clinical guidance.

  • MUR and repeat dispensing certification.
