Job summary
As a Health
& Well-Being Coach you will work as a key part of the primary care network
(PCN) multi-disciplinary team. You will consider the whole person in addressing
existing issues and encouraging the proactive prevention of new illnesses,
promoting lifestyle medicine approaches. A Health & Well-Being Coach will
support people with lower levels of patient activation to develop the
knowledge, skills, and confidence to manage their health and wellbeing, whilst
increasing their ability to access and utilise community support offers.
Main duties of the job
This is a
new and developing role. The post holders will work in partnership with their
clinical and non-clinical colleagues, management support and the wider PCN to
ensure the role delivers the best possible outcomes for our patients. The role
is varied, and may include supporting self-management education, peer support,
case management and facilitating group consultations. You will take an approach
that is non-judgemental, based on strong communication and negotiation skills.
You will support personal choice and positive risk taking, while ensuring that
patients understand the accountability of their own actions and decisions. As a
whole, your role and skills will support and encourage the prevention of
developing further illness, or the deterioration of existing long-term
conditions.

Network Central Primary Care Network (PCN) consists of 3 GP
surgeries in Barking & Dagenham. We cover a population size of over 30,000
patients.

professional and friendly multi-disciplinary team. You shall be accountable to
the Board and GP Partners.

- Level 6 qualification or working towards (M)
- Demonstrable knowledge in lifestyle or health interventions, for example exercise, nutrition (E)
- Demonstrable commitment to professional and personal development (E)
- Knowledge of the personalised care approach (E)
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers (E)
- Understanding of, and commitment to, equality, diversity, and inclusion (E)
- Knowledge of community development approaches (E)
- Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports (E)
Desirable
- Level 3 qualification in coaching, mentoring, information advice and guidance or similar.
- Knowledge of long-term conditions, for example diabetes or cardiovascular disease.
