Job summary
The post holder will work to support the existing DBTM and alongside clinical and operational teams within the Primary Care Network (PCN) to create and deliver our vision and to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource, and tools to deliver changes in a collaborative way. The post holder will support this vision to be developed in alignment with ICS strategy. They will provide complementary leadership and operational support to the existing DBTM, acting as a key point of coordination and continuity across programmes of work, ensuring alignment and delivery in their absence as required.
The post holder will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered.
The post holder will develop relationships with ICSs to gain agreement and support for local transformation plans and activity, and to support alignment with broader strategy, including the ICS's digital strategy. Working with relevant colleagues in the ICS, the post holder will also develop relationships with local delivery partners (such as IT service providers and other NHS organisations) to support effective delivery of transformation.
Main duties of the job
This expertise will be used to develop and implement a strategic plan to build and implement a robust digital and business infrastructure which:
a) Develops and manages the implementation of the PCN strategic plan in the light of changing circumstances and ensure its alignment and collaboration with the Nottingham City Place Based Partnership Plan and other local and Integrated Care System priorities
b) Communicates the work and plans of the PCN to practices, communities, and system partners to facilitate engagement and ensure that the PCN has a very strong reputation for openness, innovation, and success
c) Builds relationships and facilitate collaboration between practices to support the delivery of care to each other's patients (for example to delivery of enhanced access or other hub type working arrangements)
d) Drives integrated working that improves operational efficiency whilst reducing duplication and variation between PCN services and practices
e) Supports the financial management of the PCN ensuring that funding opportunities are maximised, and financial resources are used appropriately and effectively
f) Manages and develops the PCN workforce staff skills and capabilities both with regards to patient care and leadership reflecting staff welfare and staff experience
g) Ensures that digital and operational PCN transformation is embedded in, and aligned with, ICS and national strategies

Nottingham City General Practice Alliance (NCGPA) was formed in 2016. We are Nottingham City's first GP Federation, often referred to as NCGPA or the Alliance. Our members consist of over 40 NHS GP practices in Nottingham city, who cover a population of over 336,000 patients. We are the lead employer for our PCN based roles such as Clinical Pharmacists, Pharmacy Technicians Social Prescribing Link Workers and Health and Wellbeing Coaches.
