Job summary
Nottingham City GP Alliance is seeking a skilled and dedicated Advanced Clinical Practitioner (ACP) to join our dynamic GP surgery teams across locations in Nottingham City. The successful candidate will provide high quality care to patients across a range of conditions, working autonomously and collaboratively with GPs and the wider practice team.
Main duties of the job
Key Responsibilities include:o Assess, diagnose, and treat a wide range of acute and chronic conditions.o Prescribe medication in accordance with national and local guidelines.o Manage and review treatment plans, providing ongoing care and support to patients.o Provide health promotion, disease prevention, and patient education.o Offer consultations for minor illness and injury, with the ability to refer or escalate care when necessary.o Work closely with the clinical team to ensure coordinated patient care.
Requirements:o Registered Practitioner with a Master's degree in Advanced Practice or equivalent experience.o Independent prescriber status.o Extensive clinical experience in primary care, urgent care, or emergency medicine is desirable.o Strong communication and interpersonal skills.o Ability to work autonomously and as part of a multidisciplinary team.o This is an excellent opportunity for an experienced ACP to contribute to a forward-thinking and patient-focused practices as we develop a service model that meets the needs of our disadvantaged population.

Parliament Street Medical Centre is a city centre practice with a growing list size. It has a young population with a low rate of home visits. The practice has a history of achieving exemplary patient care (outstanding CQC rating). It has a high level of patient need around Severe Multiple Disadvantage and the existing team will work to support and develop new clinicians around this.Bilborough Medical Centre & Grange Farm Medical Centre are in the West of the City, serving a very deprived area of Nottingham. They have a more traditional population with significant health problems and complex needs. The practices are looking to develop and embed innovative multidisciplinary team working and intend to expand this further to include many of the new ARRS roles. Grange Farm provides a lead care home role within its PCN.Greendale Primary Care Centre serves patients across Sneinton and St Annes in the East of the City. Also working in an area of significant deprivation with a significant number of patients where English is not their first language, Greendale PCC is an established GP training practice, and the training leads are supporting NCGPA to develop training across our other sites.
Each of our sites have GP and management leads to support the team and they in turn are supported by the NCGPA Medical Director and Head of Practice Operations. NCGPA has a robust clinical and operational governance process to support quality and safety across the organisation.NCGPA is developing a new model of collaboration across our sites, to standardise systems, share and expand back-office functions. Our ambition is to create a new model of resilient care for the city, preserving and improving primary care services for the citizens of Nottingham.
