Job summary
The post holder will work within a PCN pharmacy team within Nottingham City to deliver and support the PCN DES and other relevant national and local medication related strategies and policies. They will also support the provision of clinical pharmacy in GP practices, care homes and the wider community with the aim of optimising patient care, improving prevention and reducing health inequalities.
The successful candidate will be a highly motivated and dynamic pharmacist who will work within one of our well-established PCN teams to improve the quality, equity and safety of care offered to their patients. They will support their PCN to implement effective medicines management, identifying areas for improvement, as well as initiating and managing change. They will also promote high quality patient-centred, cost effective and evidence-based use of medicines.
Responsibilities for all PCN pharmacistsWorking with stakeholdersRecognise the relevant key multi-professional stakeholders across the ICS to include, and not limited to, the wider health and social care system. Identify and develop good working relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to promote and share best practice. Prioritise and focus on working collaboratively with pharmacy services in other pharmacy sectors such as community pharmacy, medicines optimisation, secondary care, and mental health trust.
Main duties of the job
Maintain comprehensive documentation and records according to legal and local requirements. This includes recording all relevant pharmaceutical information in the patients record and communicating in writing with other health care professionals.Provide training / teaching / education sessions to practice healthcare professionals and administration staff including GPs, Nurses, Pharmacy Technicians and Pharmacists within the PCN.Research Look for opportunities to contribute to relevant research in collaboration with a range of stakeholders to innovate and improve patient-centred care. Leadership and managementSupport the medicines optimisation agenda across the PCN to ensure:o Safe and effective prescribing including that related to shared care.o Implementation of shared decision makingo Prompt identification and addressing of medicines related issueso Compliance with relevant medicines legislationo Delivery of antimicrobial stewardshipo Quality improvement including medicines-related digital and operational transformation.Contribute to the delivery of Network Contract DES, quality and outcomes framework and enhanced services.
Develop own knowledge and skills with a particular focus on primary care through continuous professional development activity.Be an active member of the PCN and GP practice teams by the attendance and presentation at staff meetings, training sessions, external courses, reflective practice, and clinical supervision.

We are the lead employer for our pcn based on roles such as Clinical pharmacists , pharmacy technicians , social prescribing link workers and Health and wellbeing Coaches .
