Job summary

Phyllis Tuckwell provides palliative and end of life care for people living with an advanced or terminal illness. Based in Farnham, Camberley and Guildford, we serve a population of 550,000 across West Surrey and North East Hampshire.

This a great time to join Phyllis Tuckwell as we develop services to deliver on our 2024-2029 strategy and prepare to return to a new purpose-built Hospice in Farnham.

The role requires a person with excellent skills in leading and motivating a team during this period of change, they will inspire members of the team to work cohesively, safely and effectively and will have the resilience and skills to continue to provide leadership to continuously improve services for our population. The role will support our vision, We want everyone to have the best possible experience at end of life.

Hospice at Home Manager

37.5 hours per week

Salary: £60,549 per annum WTE (plus on-call allowance)

Phyllis Tuckwell is looking to appoint a Hospice at Home Manager with experience and expertise in clinical leadership and management, a commitment to person and family centred care and to community working.

As Hospice at Home Manager, the role is both internal and external facing and supports high-quality care and seamless operational multidisciplinary team working around the patient and family.

Main duties of the job

The manager will ensure a One Team approach around the patient and family. They will develop our Community Ward and will consolidate proactive planned work in communities. They will support effective practice and learning for Clinical Nurse Specialists and all Nurses and Heath Care Assistants working within Phyllis Tuckwell Community teams. The successful candidate will be a visible, authentic leader and will promote a learning culture, teamwork and collaboration.

Planned Proactive home-based care with nursing teams in each of three localities, each led by a Senior Clinical Nurse Specialists. The locality teams work with the Multidisciplinary Team around patient needs.

For patients and families with for patients who are in an unstable phase of illness, who have rapidly changing palliative care needs or who are in their last days or short weeks of life. Includes:

Community Ward

Home night support from skilled Health Care Assistants.

A Virtual Ward - Consultant-led medical service for patients and families with heightened palliative care needs.

Living Well: site based proactive, planned palliative and supportive care for patients who are stable or slowly deteriorating, their families and carers.

This Hopsice at Home Manager role works within the Director of Patient Services Team of: Community Services Manager for Living Well and Therapies, Patient and Family Support Manager, Inpatient Unit Manager, Clinical Education Manager, Deputy Director for Nursing and Quality and as part of the Clinical Leadership Team which includes the Medical Director, Consultants in Palliative Medicine and the Clinical Administrator Manager.

This is an exciting phase for Phyllis Tuckwell, Your New Hospice project is well underway, we will occupy a new building on our Farnham site in 2026, our Hospice at Home services also operate from The Beacon Centre in Guildford and in our Communities, this role requires visible leadership and travel across the catchment area.

RESPONSIBILITIES:

1.0 Professional and Clinical Leadership

a. To lead, develop and manage the community multi-professional team ensuring high quality integrated care across service areas.

b. To foster partnership working with patients and families and integrated care around the patient.

c. Delegate effectively.

d. To develop the community model, to ensure robust and responsive service delivery.

e. To provide a visible, accessible senior clinical management presence to the teams, monitoring professional standards and effective use of resources.

f. To maintain own clinical knowledge and skills through clinical decision making and use of clinical judgement in relation to patient safety and clinical governance.

g. To keep under review clinical practice through the monitoring of quality standards, clinical supervision, clinical audit, risk management, critical incident review and reflective practice.

h. To maintain oversight of the caseloads, monitoring patient acuity and dependency, sill mix and staffing levels ensuring resources are utilised effectively, patients and family needs are prioritised according to service needs, and wellbeing of staff is supported

i. To act as the Infection Prevention and Control Lead for the community working jointly with other managers and leads, to inform clinical practice.

j. To ensure effective multidisciplinary meetings for patient and family benefit and safe, effective practice.

k. To liaise with external partners promoting relationships and partnership working, and to ensure timely and appropriate referrals.

l. To be proactive in developing our approach on inclusion and equity enabling access for all who need us.

m. To promote the highest standards of holistic clinical care are delivered and the use of compassionate, clear, and sensitive communication.

n. To ensure patient records are properly maintained in accordance with professional guidelines and Phyllis Tuckwell policy.

o. To be aware of current thinking, innovations and evidence base for community service care provision.

p. To ensure knowledge about patients is maintained in the strictest confidence and is not divulged except to professionals, where it would materially assist with the management of care or protection of the individual. (See Staff Handbook Confidentiality and Data Protection Act).

q. To ensure that the standards set by the Care Quality Commission, NICE guidance, other national bodies and relevant safety notices are adhered to in accordance with legal requirements.

b. To promote a culture of psychologically safe where colleagues wellbeing is promoted and supported.

e. To investigate complaints, incidents or concerns from patients or their relatives, reporting outcomes of the investigation to the Director of Patient Services/Medical Director, Clinical Governance, and as appropriate, to external organisations e.g. Care Quality Commission and Safeguarding.

f. To effectively participate in and facilitate patient safety and quality improvement groups, to write and update relevant policies, procedures, and guidelines.

g. To help develop operational plans for an effective and efficient clinical service to patients and their families/carers.

h. To seek patient feedback and develop patient and family engagement in improving our provision.

i. To contribute to the annual business planning process.

j. To be responsible for clinical budgets related to the line managed areas, working with the Director of Patient Services on preparations for budget setting.
