Phyllis Tuckwell provides supportive and end of life care for people living with an advanced or terminal illness. Based in Farnham and Guildford, we serve a population of 550,000 across West Surrey and North East Hampshire.

Senior Registered Nurse (Night Support Community)

Salary £39,433 per annum WTE, plus enhancements

Full time and part time hours available.

All applicants must be able to work a minimum of one night per week from 8:30pm to 7:30am with an opportunity to increase number of nights to cover leave and one 9.5 hours day per week

Phyllis Tuckwell is recruiting a Night Support Senior Registered Nurse to work between hours of 8:30 and 7:30 one night a week, with remaining hours to be worked during the day. There will be an opportunity to work additional nights to cover for leave. The role is community facing, within our Hospice Care at Home team.

Main duties of the job

The role involves responding to the needs of patients known to Phyllis Tuckwell, including providing telephone advice and visits, these are often in response to a patients rapidly changing needs and for symptom control. The role dovetails with out of hours community nursing ensuring there is a team approach to patient care.

This is a senior nursing role and is an integral member of the whole Phyllis Tuckwell Team, providing person centred care, as such there is a need to work closely with the day service, and to provide nursing care and support on the In-Patient Unit (IPU) when this is required.

We are looking for a proactive, experienced, registered nurse who is familiar with working in the community, has experience of providing palliative and end of life care and medicines management

The successful candidate will have:

  • Experience as a senior community nurse with strong assessment skills relevant to palliative and end of life care.
  • Excellent clinical, decision making and assessment skills
  • Excellent communication and interpersonal skills
  • Symptom management experience and willing to undertake relevant competencies.
  • An awareness of risk assessment and management

Job Purpose:

The postholder will maintain the highest standards of nursing care with patients, their families and carers This is primarily a community facing role and will involve working overnight and day shifts to provide holistic assessment, identification of patient needs, care planning and care provision jointly with other multidisciplinary team members, i.e.colleagues at Phyllis Tuckwell Hospice and with our community partners. The service is complementary to, and dovetails with, the community nursing services in Guildford and Waverley, North East Hampshire and Farnham and Surrey Heath so that people who use our service receive co-ordinated and person centred care.

The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective and efficient culture

by participating in risk assessment, risk management, audit and quality improvement initiatives and by working in accordance with PTH policies and procedures. They will facilitate the support and professional development of colleagues, health care assistants and students.

The RN Night Support role encompasses providing back up and support for the IPU overnight, should there be unscheduled absence of the RN during the night.

Responsibilities

1.0 Clinical

1.1 To provide clinical guidance and support to staff

1.2 To ensure statutory obligations are met and staff follow those relevant to the clinical area.

1.3 To ensure quality standards are maintained and constantly re-evaluated in order to provide the highest standards of holistic care.

1.4 To co-ordinate and maintain 24-hour provision of assessment, planning, implementation and evaluation of nursing care in conjunction with the District Nurse assessment in the community setting.

1.5 To work as part of the multi-disciplinary team, providing holistic care to the patients, their families and carers.

1.6 To ensure patient records are properly maintained in accordance with NMC guidelines and PTH policy.

1.7 To be able to demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace.

1.8 To report accidents, incidents and complaints to the team lead/ line manager same day.

1.9 To understand and adhere to the PTH and Community medicines management policy in relation to the correct handling, usage, and administration of all drugs.

2.1 To understand and work within the National Care Standards Act 2000, the PTH business plan and philosophy.

2.2 To provide appropriate information and support to patients, carers and families.

2.3 To liaise and develop effective relationships with external health care professionals and other agencies, ensuring continuity across all care settings.

3.1 To ensure a safe working environment is maintained, in accordance with the PTH Care Fire and Health and Safety policies, reporting any potential hazards to the line manager.

3.3 To ensure that the team has the appropriate skill mix and nursing resources are used wisely.
