Job summary
The
Sandwell Community Dementia Support Service (SCDSS) is commissioned by Sandwell
Metropolitan Council and the Black Country and West Birmingham Clinical
Commissioning Group. As lead provider, we are working alongside seven other
local organisations; Agewell CIC, Dementia Pathfinders CIC, West Bromwich
African Caribbean Resource Centre, St Albans Community Association, BUDS, Age
UK Sandwell and Alzheimers Society to provide this service across Sandwell
Metropolitan Borough.
Main duties of the job
As a Community Dementia Adviser, you will be required to
provide relevant information in a variety of formats to meet individual needs
as well as signposting and/or referring them to appropriate services as well as
encouraging engagement in community activities.

- Knowledge of dementia, person-centred approaches, issues that affect family carers and availability of local services.
- Experience of working in a busy environment and supporting others in a team.
- Record-keeping skills and writing ability of a high standard suitable for written communication with service users and professionals, production of publicity materials and reports.
- Excellent IT skills with confident use of MS Office packages including Word, Excel, Outlook and Power Point.
- An understanding of client confidentiality (GDPR).
- Full current UK driving licence and access to a vehicle to travel independently across the service area.
- Knowledge of and ability to work to Adult Safeguarding Procedures, Lone Working Procedures and the Mental Capacity Act.
