Job summary

In line with Alternative Futures Group mission, vision and values, the job holder will provide effective clinical leadership and support to achieve the delivery of community rehabilitation within AFGs mental health supported living footprint.

The post holder will achieve this by supporting Team Leaders & Area Managers to provide services that are needs led, compliant with national guidance and viewed positively by people supported.

Main duties of the job

  • Enable people to work towards their recovery, and independent living that is meaningful and safe. This means living within their local area in an environment that is least restrictive, is recovery focussed and delivers psychologically informed care that is coproduced wherever possible.
  • Provide coaching and supervision to mental health supported living staff thereby enabling them to support individuals with complex needs in their rehabilitation and recovery pathway.
  • Work collaboratively alongside staff teams across AFG mental health supportive living, people supported, and local multi-agency professionals.
  • To understand and value others roles and contributions. Enable the team to contribute effectively, acknowledging the nature of their work and sharing their own knowledge and skills to provide recovery focused support in the least restrictive environment.
  • Provide effective liaison between other professionals, agencies, and families to provide the best possible outcomes.
  • Offer support and guidance to staff teams in Community and Adult Care Home Services by coaching and supervising staff as required in delivering interventions within a psychosocial and person-centred framework.

  • Work across a 7-day week working 9-5 to provide support to Team Leaders in AFGs Community and Adult care Home Services across the North West.

Audit & Training

  • Participate in audit reviews and other initiatives in accordance with AFG business objectives and Quality Compliance Committee.
  • Follow AFGs policies and procedures when coaching and mentoring teams, reporting any performance issues to the line manager.
  • Facilitate teaching and educational sessions to staff teams with particular emphasis on relapse prevention, mental health awareness, MHA and psychosocial approaches.

Clinical effectiveness and coaching

  • Promote professional values and standards when working with other professionals, families, and carers.
  • Support the team to embed psychosocial and person-centred approaches within their care files
  • Support teams to take therapeutic risk by ensuring a full and comprehensive assessment of risk has been carried out by the referring agency. Translate this into a formulation to drive risk management plans.
  • Support teams to prepare reports for CPA meetings and support people supported to attend. Attend CPA meetings to support the team leaders.
  • Support delivery of care that is person centred and reflects current best practice, challenging any practices that is seen as detrimental to the person supported.
  • Support the team to implement, monitor and review Service Users personalised plans, taking account any CPA plans.
  • Ensure effective supervision of teams working with complex cases that includes learning from events.

  • Take responsibility for own continuous professional development and engage in clinical supervision.
  • Actively participate in performance reviews with Line Manager and attend mandatory training as required.

  • This role is home based but may require some travel within the north west of England on occasion - a driver with the use of a vehicle is therefore required.

  • Degree level education in related field
