Paragon Home Healthcare is seeking Female Complex Homecare Nurse to support a female client who requires ongoing nursing support at home. This role involves caring for a client with a combination of complex long-term conditions, including chronic pancreatitis, hypermobility/Ehlers-Danlos features, chronic pain and neurodivergent needs. She has been in hospital for an extended period, and nurses will play a key part in supporting a safe and well- planned transition home.
Main duties of the job
You'll be delivering person-centred, holistic care such as complex medication management (including subcutaneous and jejunostomy-administered medications), bowel care, respiratory support and safe mobility. The client benefits from calm, structured communication and consistent routines, and you'll be supported by clear protocols to guide practice.
You'll play a vital role in promoting health, safety and quality of life within the home environment. Working closely with our Nurse Practitioner team, you'll help ensure her care plan is followed smoothly and contribute to a stable, supportive model of care that encourages confidence, independence and overall well-being.
