Job responsibilities
Courtyard
Care Ltd will endeavor to provide a family orientated, negotiated care package
to meet the needs of all the children and their families.
This
job is within the Children's homes to lead in the on-going
assessment, management and delivery of care packages for children and young
people receiving complex health needs. including setting and maintaining
standards of care; professional advice (where appropriate), customer liaison,training
and supervision of support workers and team development. To ensure service
meets and exceeds all relevant legislative/commissioning frameworks.
Key
responsibilities:
Training and development
Responsible for training new staff in all
relevant health needs for each child, supporting and supervising new staff from
a clinical perspective but not being allocated to or responsible for the child/young
person
Complete refresher updates and
competencies for all staff as per Courtyard Care Policies
Assist in the development for on going
competency assessments for all staff
Develop training resources and materials
Deliver training in mandatory areas such
as medication, first aid etc
Clinical
Be responsible for own ongoing
professional development
Identify and
assess care need requirements within a home care package and maintain regular
and appropriate contact with the Registered Manager to ensure appropriate
levels of staffing are met.
Ensure that
clinical interventions are met through effective planning and use of staff
Undertake
six Monthly review/update of health needs plan for child/young person.
Ensure that
all intervention are within the relevant framework
Provide
clinical interface between the service, commissioners, line managers attend all
childrens reviews and related meetings as identified by the registered Manager
Provide
support and guidance to the staff team for all matters of a clinical nature
Work within
NMC guidelines
Develop and
undertake medication audits in services to ensure highest standards of practice
Maintain
effective Registration with NM
Lead by
example through attention to professional conduct
Attend team
meetings for clinical issues/matters only
Support the
Registered Manager in the root cause analysis of and leaning outcomes following
medication errors
Contribute to
clinical policy update and review
Advisors
Practice
educator will offer advice to team in clinical matters within the health
support plan and direct them to universal services outside of the health
support plan
Liaise with
health professionals to provide clinical oversight from in service.
Person
specification
A core profession a nurse practitioner.
Member of the relevant regulatory body
with no ongoing disciplinary action or investigations pending. Up to date CPD
profile.
RSCN - Registered Nurse with paediatric
experience
ENB 998 or equivalent experience
Post Registration qualification
Experience of ITU or High Dependency
unit
Critical care management
Effective communicator, organised and
pro-active
Awareness of current clinical legislation
Experience of working in a
multi-disciplinary team
Flexible, self-motivated, team player and
ability to problem solve
Willingness and ability to maintain all
your clinical documentation in a manner consistent with GDPR legislation,
Courtyard Care Ltd policy and the guidance of your regulatory body.
General:
Good knowledge of computer system
Work as an effective member of a
multi-disciplinary team creating positive working relationships.
All annual leave will be booked in
accordance with Courtyard Care Ltd Policy and accurate records to support the
Registered Manager.
Ensure appropriate procedures are in
place for evidencing the training for the staff team.
You are expected to report on duty at
commencement time of shift, any lateness must be reported to management and
documented on the rota and in the daily log.
Any sickness should be conducted as
per policy and a back to work form must be completed on return to work. Any
sickness of staff must be monitored and dealt with in line with company policy.

to all staff, children / young people and visitors
Dress
Code
Staff
should dress appropriately for the work and activities they undertake in their
day to day roles.
Policies and Procedures
All
staff are required to undertake an induction at which a number of policies and
procedures will be detailed and other will be signposted to be reviewed during
your probationary period and subsequently your supervisions which will be held
on a regular basis with your line manager.
Confidentiality
and Data Protection
All
staff must maintain confidentiality of information about children and young
people and staff and meet the needs of the Data Protection Act (1998) at all
times. Staff must follow the Courtyard Care Ltd Policies and Procedures in this
area. Any confidential information gain by a member of staff in their role must
not be communicated to other persons except where required in the recognized
course of duty.
Attendance
at Meetings
- As
part of the communication processes in place to keep all staff informed and up
to date about matters relating to their jobs and employment staff will be
required to attend meetings within their own time and working time, reasonable
notice of these meetings will be given, where this is not possible this will be
due to exceptional circumstances.
Attendance at any meetings involving the well-being of children / young
people as required in line with legislation and good practice.
Continuous
Professional Development
- All
staff have a responsibility to continue their professional learning and
development that supports them in carrying out their day to day roles. Staff
will be supported in this through supervision, appraisal and staff development
processes.
