Main duties of the job

We are seeking a dynamic and compassionateClinical Lead Community Nurseto spearhead our nurse-led service. This role is ideal for a registered nurse with a passion for community care, clinical leadership, and staff development.

You'll be instrumental in triaging care concerns, supporting care teams, and ensuring our clients receive timely, responsive, and high-quality care at home.

Key Responsibilities

  • Lead the delivery of anurse-led community care model, ensuring clinical excellence and continuity of care
  • Triage care concernsraised by staff, families, or clients, and implement timely interventions to prevent deterioration
  • Provideclinical training, mentorship, and supervisionto care staff, building their confidence and competence
  • Develop and maintainclinical protocols and pathwaysto support proactive care planning
  • Collaborate with GPs, district nurses, and other health professionals to ensure integrated care delivery
  • Monitor outcomes and contribute toreducing hospital admissionsthrough early intervention and preventative care
  • Champion a culture ofclinical accountability, safety, and compassionacross the service
  • Participate in audits, quality improvement initiatives, and service development
