Main duties of the job

We'd like you to join us as a Discharge Co-ordinator at our Frenchay Brain Injury Rehabilitation Centre in Bristol. At Frenchay Brain Injury Rehabilitation Centre we work to enable patients to achieve clearly defined and agreed goals. What you'll be working:

What you'll be doing:

As the Discharge Co-ordinator you will ensure patients at the Frenchay Brain Injury Rehabilitation Centre have well-coordinated discharge care packages alongside effective, seamless discharge plans. To be an active member of the inter-disciplinary team, and provide a comprehensive community liaison service with other health professionals, social services and external agencies. To provide specialist advice and support to patients, carers and relevant external agencies around discharge issues.

  • To lead the coordination of discharge planning for patients with complex needs, taking responsibility as appropriate for liaising and maintaining contact with families and external agencies to ensure safe and effective patient discharge
  • To be able to gather and collate all relevant information pertaining to patients into an accessible format and share with appropriate community partners in order to help identify appropriate discharge destinations/care packages
  • Liaise where appropriate with the external agencies such as Social Services and Continuing Health Care team, ensuring that patient referrals and documentation are received in the required time scales.
  • Build and sustain strong relationships with the range of community and primary care services, including social services, intermediate care, community matrons, voluntary sector and private sector to ensure patients receive a safe and effective discharge plan.
  • Support the process of finding and securing appropriate placements for patients in conjunction with Social Services (to include visits as appropriate).
  • Demonstrate understanding of policy and protocols that underpin the discharge processes
  • To give verbal updates and coordinate written reports to health and social care staff and other appropriate professionals on the patient’s progress.
  • Respond to information received regarding discharge destination breakdown and liaise with relevant parties to ensure resolution, taking action where appropriate to develop and improve practice.
  • To attend multi professional meetings and case conferences to provide expert discharge advice in order to facilitate a timely appropriate discharge.
  • Take professional responsibility for ensuring effective communication between all services providers across the discharge pathway.

  • A current NMC registration RN/ therapist
  • A legal right to work in the UK
  • 2 years experience of working within the discharge process, including working with patients who have complex presentations and social situations
  • Experience of multi-professional working within the Health and Social Care community
  • Excellent negotiation and people management skills
  • Experience of online record keeping systems
  • Effective and efficient team working and able to work autonomously
  • Excellent communication skills, verbal, nonverbal and written.
  • Effective time management skills
  • Skills in collating and prioritising information to formulate effective discharge plans
  • Excellent negotiation and people management skills
  • Experience of online record keeping systems
  • Extensive experience of providing clinical care to complex and dependent patients to a consistently high standard
  • Experience of change management and new ways of working, to deliver care in response to a service expansion or change
  • Experience working within a MDT within a complex pathway

  • Experience of undertaking staff supervision/coaching as a means of promoting staff development
