Job summary

This role is for a Team Leader - Nights at Avery Healthcare's Wandsworth Common Care Home in London. The successful candidate will be responsible for supporting the home management team in delivering high-quality, person-centred care to the residents.

Main duties of the job

As a Team Leader - Nights, your focus will be on providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Key responsibilities include leading the care planning process, assisting with resident discharges, monitoring resident well-being, and ensuring safe systems of work. You will need to have an NVQ 3 or equivalent in care for care homes, two years' experience in a senior position, and the ability to communicate effectively both verbally and in writing.

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.

Other responsibilitieswillinclude:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
