Job summary
This role as a Team Leader at Avery Healthcare offers the opportunity to support the home management team in delivering high-quality, person-centered care for the residents. Main duties of the job
As a Team Leader, your focus will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Key responsibilities include leading in the person-centered care planning process, assisting in the smooth transition of discharges, monitoring the well-being of each resident, and practicing safe systems of work.
