An Obstetrician and Gynaecologist provides inpatient services, outpatient gynaecology clinics, and procedural services, including colposcopy, which is a core component of the national cervical cancer prevention and screening programme.

The Obstetrician and Gynaecologist will provide strategic, clinical, and advisory leadership in strengthening maternal, newborn and women's health services across primary and secondary care.

Montserrat, referred to as the Emerald Isle of the Caribbean is an overseas territory of the United Kingdom, located in the Lesser Antilles chain and has a land mass of 39.5 square miles.
The delivery of public health care on Montserrat is the responsibility of the Government and is administered under the ambit of the Ministry of Health and Social Services (MoHSS) with a staff complement of two hundred and forty (240). Construction of a purpose-built hospital began in April 2025 and is scheduled to be completed in April 2027.

Clinical
- Be responsible for the assessment and lead the clinical management of all patients admitted to the Obstetrics Ward, including the conduct of daily ward rounds, emergency care, and participation in the on-call rota, ensuring safe, timely, and evidence-based care.
- Consult and collaborate with the Physician Specialist, Surgeon Specialist, Anaesthesiologist, Paediatrician, and other relevant clinicians regarding all admissions to the Female Ward involving obstetric or gynaecological conditions, ensuring coordinated multidisciplinary management.
- Conduct outpatient clinics, including but not limited to High-Risk Pregnancy Clinics, Antenatal Clinics, Adolescent Clinics, and Well-Woman Clinics, at both Primary and Secondary Health Care facilities, in accordance with national policies and clinical pathways.
- Participate in, initiate, and lead continuing medical and nursing education sessions. Make evidence-based recommendations and provide clinical leadership in the development, improvement, and implementation of screening programmes affecting womens health, including breast and cervical cancer screening, taking into account available resources.
- Participate in health education and health promotion programmes aimed at improving maternal, newborn, and womens health outcomes at both the individual and community levels.
- Work closely with clinical and non-clinical staff to develop, implement, and maintain systems that enhance patient safety, improve quality of care, and minimise the risk of adverse maternal and gynaecological outcomes, including participation in clinical audits and quality improvement initiatives.
- Contribute to the finalisation and ongoing review of the Essential Drugs List. Submit justified requests for the procurement of specialised medications, medical supplies, and equipment not routinely stocked, for approval by the Chief Medical Officer prior to submission to the Senior Pharmacist, Storekeeper, and Director of Secondary Care, as applicable.
- Provide general medical services in Casualty and Health Centres when assigned. The post holder should expect that up to 20% of overall clinical time may be allocated to the provision of general medical services, reflecting the service needs of a small-island health system.
Administrative
- Participate in departmental and hospital management meetings.
- Provide supervision and performance feedback to midwives.
- Coordinate overseas referrals in collaboration with relevant stakeholders. Advisory - The Obstetrician and Gynaecologist will provide strategic, clinical, and advisory leadership in strengthening Maternal, Newborn, and Womens Health services across primary and secondary care.
Quality Improvement Programme Maternal and Newborn Care
The post holder will lead the design and delivery of a structured quality improvement programme for maternal and newborn care at both primary and secondary care levels, implemented in the following phases: Phase 1 (time frame: 6 months)
- Conduct a participatory gap analysis involving all relevant healthcare professionals engaged in the delivery of maternity and newborn care, with the aim of identifying strengths, weaknesses, risks, and opportunities within existing services.
- Assess and make recommendations to strengthen linkages between primary and secondary care, particularly in relation to antenatal care, intrapartum management, and postnatal follow-up.
- Map existing clinical pathways and identify opportunities for service optimisation, including improvements in referral processes, escalation of care, and the management of overseas referrals for tertiary-level and/or neonatal intensive care services.
- Review existing clinical protocols and guidelines related to obstetric and newborn care against recognised regional and international standards, including WHO and UK guidance, ensuring relevance to the Montserrat context.

- Medical Degree (MBBS, MD, or equivalent) from a recognised university
- Specialist qualification in Obstetrics and Gynaecology from a recognised institution
- Full registration with a recognised national or state medical regulatory authority
- Demonstrated evidence and experience in performing and interpreting obstetric ultrasounds and electronic fetal monitoring
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
