Job summary

Job Overview

We are seeking a skilled and compassionate Perinatal Mental Health Nurse to join our specialist perinatal mental health service in Jersey. This role focuses on supporting women during pregnancy and up to two years postnatally who are experiencing moderate to several mental health difficulties.

You will work as part of a skilled and enthusiastic multidisciplinary team, providing evidence-based assessment, care planning, and therapeutic interventions, while promoting recovery, safeguarding infants, and supporting families during this critical period. You will be a motivated and empathetic nurse with a strong interest or background in perinatal mental health.

  9. Mentors and supports students and junior members of staff as and when required.

Mental health services in Jersey are undergoing significant change. This is an exciting opportunity to be part of a small specialist Community mental health team providing Mental Health Care to the people of Jersey.

Job responsibilities

To promote and develop the role of mental health liaison by providing clinical and professional leadership in development and implementation of evidence-based practices to support people with a mental health need who are under the care of the General Hospital. To facilitate high-quality person-centred specialist health care for people with mental health needs within a hospital setting. To ensure seamless care provision between mainstream hospital wards and mental health services including inpatient and community services. Providing nursing advice, education and direct support on complex health care needs of people with a mental health need, families, staff and provider services.

Responsible for liaising with Hospital wards, Adult Social Care & Mental Health Service staff and all other relevant carers in order to facilitate an effective health care pathway for people with mental health needs who have been admitted to the JGH.

Enhance care delivery by promoting a whole system approach to achieve positive outcomes within a multi-agency/disciplinary partnership.

Participate in multidisciplinary/agency team meetings, hospital admissions, case conferences, pre discharge meetings, as well as referrals onwards to other mental health teams.

Engage with all health care services to promote the rights of individuals, to access an equitable service and reasonable adjustments are made in order to meet individual needs.

Participate in the development and review of policies and procedures to ensure the health needs of people with mental health needs are considered and in particular, to develop processes and pathways within referrals and admissions to health care services.

Liaise with Hospital, Acute Services and GP practices to ensure appropriate needs identified and recorded, to carry out routine mental health assessment, screening and consultation for adults with a mental health need where required.

Assess and advise on issues surrounding complex health care needs of people with mental health in relation to emergency or elective admission, access to investigations and diagnostics required for health care needs and for discharge planning.

Provide mental health awareness training to staff from Hospital, other health professions, students.

Provide knowledge and advice to care staff on complex health needs and presentations (e.g. dementia, psychotic disorders)

Maintain as part of continual professional development up to date clinical mental health evidenced based knowledge/skills, using information to affect change in practice and ensure dissemination of new knowledge working within equality and diversity guidance, to reduce discrimination and disadvantage by making access to health services equitable.
