Job summary

To promote and develop the role of mental health liaison by providing clinical and professional leadership in development and implementation of evidence-based practices to support people with a mental health need.

To coordinate and facilitate high-quality person-centred specialist health care for people with mental health needs within a hospital setting.

To ensure seamless care provision between mainstream Hospital services, Primary Care services, GP surgeries and specialist learning disability services to reduce health inequalities.

Main duties of the job

1. Responsible for liaising with Hospital wards, Adult social care & Mental health staff and all other relevant carers in order to facilitate an effective health care pathway for people with mental health needs.

2. Enhance care delivery by promoting a whole system approach to achieve positive outcomes within a multi-agency/disciplinary partnership.

3. Co-ordinate and Participate in multidisciplinary team meetings, hospital admissions, case conferences, pre discharge meetings aswell as referrals to other mental health teams.

4. Engage with all health care services to promote the rights of individuals with mental health needs to access an equitable service and reasonable adjustments are made in order to meet individual needs.

5. Participate in the development and review of policies and procedures to ensure the health needs of people with Mental health needs are considered and in particular, to develop processes and pathways within referrals and admissions to health care services.

6. Liaise with Hospital, Acute Services and GP practices to ensure people with mental health needs are appropriately identified and recorded, supporting staff to develop competencies to carry out routine health assessment, screening and consultation for adults with a mental health need where required.

  • - Driving licence with own vehicle.
  • - Work based skills (Dementia, syndrome specific, Catheter, PEG, Maybo competent) or willingness to develop in post.
  • - Skilled in independently coordinating clients in an agreed caseload.
  • - Excellent communication and report writing skills, Typing/IT skills.
  • - Adaptable and flexible to meet changing needs.
  • - Flexibility in approach to the role, striving to look for solutions which
  • meet the needs of individuals.
  • - Emotional resilience and maturity.
  • - Objectivity and fairness.

Desirable

  • - Knowledge of behavioural methodology (Positive Behaviour Support).
  • - Second language desirable
  • - Conflict resolution Communicates effectively across all levels.
