Job responsibilities
The post is based within the Jersey
Mental Health Care Group.
Jersey Mental Health Care Group has
many components: -
(a) In-patient Units (Orchard House, Cedar
Ward and Beech Ward at St. Saviours Hospital Site and Clinic Pinel).
(b) Orchard
House consists of 17 adult acute beds (including an extra-care area). It is for
people aged between 18 and 65, who suffer from severe mental illness and
require inpatient admission. Our aim is to help services users achieve their
potential by promoting recovery and maintaining wellbeing. Our service users
come from a variety of cultural backgrounds and often have challenging and
complex needs.The inpatient team is a multidisciplinary team, which
includes doctors (consultant, Staff grade/ Clinical Fellow and F2), a clinical
psychologist, nurses, a social worker, occupational therapists, an art
therapist, health care assistants and administrative staff. Independent Advocacy is provided by My
Voice. The wards work closely with third sector charities such as MIND and
Focus on Mental Illness.
The
Consultant medical support to Cedar and Beech wards (which contain acute beds
for functional and organic older adults) is provided by the Older adult
community consultants. However, there is an inpatient staff grade psychiatrist/
Clinical Fellow and an F1 doctor based on the wards.
The
inpatient units are staffed by a multi-disciplinary team including
psychologists, nurses, occupational therapists, an art therapist, and a
psychiatric social worker.
The
inpatient wards will be moving to a new facility at Clinic Pinel in Autumn
2023. This will include an Article 36 suite.
(c)
Crisis and Assessment team (team base
at La Chasse in St Helier)
The
CAT team work 24/7 and provide the single point of entry into adult and older
peoples mental health services. They conduct emergency, urgent, and routine
assessments in the community, at La Chasse and in the Emergency Department and
can provide a medical review. They work closely with the police and respond to
Adult Protection Notices. The Authorised officers are part of this team and
they co-ordinate the MH law assessments. There is a multidisciplinary team of
assessors with experience in adult and older peoples psychiatry. After
assessment people will be directed to the appropriate service for their needs.
(d) Primary
Care Mental Health Service (team base Eagle House)
This
service is multidisciplinary and provides 6 session interventions for mild to
moderate mental health conditions that can be managed in primary care. It is
not a prescribing service.
(e) Adult Community Mental Health Team (team
base at Le Bas Centre in St. Helier; outpatient clinics at La Chasse in St
Helier).
The Community Mental Health Team
focuses on service users with severe and enduring mental illness. They do not conduct any initial assessments. The
consultants and staff grades who work there also provide a general psychiatric
out-patient clinic. The team includes: four consultant psychiatrists (two
part-time), two whole time equivalent staff grades, a consultant psychologist,
a team leader, nurses, occupational therapists, social workers, and health care
assistants. The team deliver a range of clinical interventions for service
users and their families including case management, Decider Skills, Cognitive
Behavioural Therapy, Dialectical Behavioural Therapy, and Eye Movement
Desensitisation and Reprocessing. The team also hosts a community support
service that offers daytime activities and supported accommodation (The Diner
and Clairvale House) to people with severe mental illness. There are wellbeing clinics for people on
depot medication and clozapine.
(f)
Home Treatment Team (team base at
Masion du Lac, St Saviours Hospital site)
The
Home Treatment Team offers intensive care to patients in their own home to
reduce admissions and facilitate early discharge from hospital. The team
includes: a consultant, a team manager, nurses, occupational therapists, health
care assistants and social workers. The
team works closely with the ward, the CMHT, and the new Crisis and Assessment Team.
(g)
Liaison (team base Jersey General
Hospital)
The Liaison Psychiatry team do not cover the Emergency Department but
provide assessment, reviews and support to Jersey General Hospital wards and
support quality improvement projects around the care of people with mental
health problems in hospital and under hospital outpatient service. It is
staffed by a team of dedicated nurse practitioners, and it will have a
dedicated Consultant Psychiatrist under the new redesign. The Liaison service
covers all adult ages. The service is under the Psychiatric Liaison
Accreditation Network (PLAN).

Older Adults Community Mental Health
Service (team base The Poplars, Overdale Hospital Site, St Helier)
The Older Adults Community Mental Health Service focuses on service users with severe
and enduring mental illness (including dementia with behavioural or psychiatric
complications). The team includes: two whole time equivalent consultant
psychiatrists, one whole time equivalent staff grade, a consultant psychologist,
a team leader, nurses, occupational therapists, social workers, and health care
assistants. The team deliver a range of clinical interventions for service
users and their families.
There is also a comprehensive memory
assessment service led by Dr. Juan Melendez (Associate Specialist)
(i)
Drug
and Alcohol Service (team base Maison du Pape, St Helier)
This
service accepts self-referrals and service referrals for people requiring
support with drug or alcohol problems. The team consists a Service Manager, a
Clinical lead Manager, a Consultant Psychiatrist (Dr Tanya Englebrecht), 3
Substance Misuse Practitioners, an Advanced Clinical Practitioner / Nurse
Prescriber, a Harm Reduction Worker / Needle Exchange Worker, a Social Worker,
an Arrest Referral Worker / Court Diversion Officer, a part time Substance
Misuse Probation Officer, a Younger Persons Substance Misuse Worker, Alcohol
Liaison Nurses, Community Detox Nurses, a Psychologist, a Counselling Student
(supervised by Psychologist), a GP for 1 session per week and admin staff. They
provide a range of treatments and interact closely with the third sector.

Jersey
Talking Therapies
Jersey
talking therapies provides a range of psychological interventions in line with
NICE guidance. Steps 2-3 are accessible by self-referral and Step 4 (PATS
service) work more closely with JAMHS and take people through internal referral
with more complex needs.
(k)
Physical Health Facilitation team
This
is a new service through our redesign process to provide annual physical health
checks and signposting to other services for health and wellbeing for service
users with severe and enduring mental health needs.
(l)
Legislation
team
This
team is made up of the people who administer the mental health and capacity law
in Jersey. It also contains the Authorised officers who undertake the mental
health and capacity law assessments.
(m) Specialist Clinics and Pathways
JAMHS
offers the following specialist services, each of which is supported by a
consultant psychiatrist or Associate Specialist with a special interest in the
area:-

Prison In-reach (Dr Brad Hillier)
Neurodevelopmental service ADHD, LD and Autism (Professor
Ulrich Muller-Sedgewick & Dr Ashutosh Kaushal)
Gender Identity Clinic (Dr Zainab Kadim)
CAMHS transition pathway (Professor Max Marshall).
This is a full-time post of 40
hours (10 PAs) per week, based in the Community Mental Health Team in St.Helier.
Work commitments and intensity will be reviewed at least annually through the
job planning process. Any substantial changes in the nature of the post will be
agreed through the job planning process. The post holders job plan
will be reviewed annually and will be assessed according to current
guidance. The postholder will be
managerially responsible to their supervising consultant and through them to
the Lead Consultant for the service.
The job plan will consist of 9
Clinical Programmed Activities (including administration) and 1 Supporting
Professional Activity (SPA). The Clinical PAs will include: direct clinical
contact (in outpatients, in the community and occasionally in hospital settings),
indirect clinical contact (in multi-disciplinary meetings and reviews), and
clinical administration. The SPA will include: CPD, audit & quality improvement
activity, appraisal, revalidation and PDP groups. The balance of PAs and SPAs may vary by
agreement between the organisation and the post holder but will not be less
than one SPA. The post holder will have
the option to participate in a speciality clinic supported by one of the
consultants.
The post holder will be
provided with a shared office space including telephone and desktop. A laptop and mobile phone will also be
provided. Administrative support is also available through the team secretary.
