Job summary

Older Adult Inpatient Services were completely refurbished in 2022. Staffing ratios and bed occupancy rates are very favourable compared with the UK.

This role also has a community component within the Community Mental Health Service. Again, there is significant ongoing investment in community services including a new all age assessment, crisis and home treatment service to support the community team. Caseload numbers and staffing are again very manageable compared with the UK.

Main duties of the job

The successful applicant will be expected to provide medical expertise in the field of older adult psychiatry with a complementary sub-specialist interest.

Participating in clinical governance activities including audit.

Providing clinical leadership, support and advice to the older adult multi-disciplinary team.

Participating in the all-age 1:10 on-call rota for psychiatry.

Job description

Job responsibilities

The post is based within the Jersey Mental Health Care Group.

Jersey Mental Health Care Group has many components: -

(a) In-patient Units (Orchard House, Cedar Ward and Beech Ward at St. Saviours).

Orchard House consists of 17 adult acute beds (including an extra-care area). Orchard House has a dedicated in-patient Consultant Psychiatrist, Staff grade and an F2 doctor.

The Consultant medical support to Cedar and Beech wards (which contain acute beds for functional and organic older adults) is provided by the Older adult community consultants. However there is an inpatient staff grade psychiatrist and an F1 doctor based on the wards.

The inpatient units are staffed by a multi-disciplinary team including psychologists, nurses, occupational therapists and a psychiatric social worker. Independent Advocacy is provided by My Voice. The wards work closely with third sector charities such as MIND and Focus on Mental Illness.

The wards will soon be combined into an inpatient unit at Clinic Pinel for a short time before there is new provision at the new build Jersey General Hospital

(b) Jersey Adult Community Mental Health Team (at La Chasse in St. Helier).

The Community Mental Health Team focuses on service users with severe and enduring mental illness. The consultants and staff grades who work there also provide a general psychiatric out-patient clinic. The team includes: four consultant psychiatrists (two part-time), two whole time equivalent staff grades, a consultant psychologist, a team leader, nurses, occupational therapists, social workers and health care assistants. The team deliver a range of clinical interventions for service users and their families including: case management, Decider Skills, Cognitive Behavioural Therapy, Dialectical Behavioural Therapy, and Eye Movement Desensitisation and Reprocessing. The team also hosts a community support service that offers daytime activities and supported accommodation (The Diner and Clairvale House) to people with severe mental illness. There are wellbeing clinics for people on depot medication and clozapine.

(c) Home Treatment Team

The Home Treatment Team offers intensive care to patients in their own home in order to reduce admission and facilitate early discharge from hospital. The team includes: a team manager (social worker), consultant, nurses, occupational therapists, health care assistants and social workers. The team works closely with the ward, the CMHT and Liaison & Street Triage.

(d) Liaison & Community Triage

Liaison & Community Triage is led by Dr. Rachel Ruddy an experienced Consultant liaison psychiatrist. It includes a Psychological Medicine Clinic that has developed good links with general medical services, including Neurology and the Pain Clinic. It is staffed by a team of dedicated nurse practitioners and mental health social workers. The Liaison service covers all adult ages.

(e) Older Adults Community Mental Health Service

The older adults community mental health service focuses on service users with severe and enduring mental illness (including dementia with behavioural or psychiatric complications. They are based at Overdale, St Helier. The team includes: two whole time equivalent consultant psychiatrists (Dr Tamal De and vacant post), one whole time equivalent staff grade, a consultant psychologist, a team leader, nurses, occupational therapists, social workers and health care assistants. The team deliver a range of clinical interventions for service users and their families, including a Primary Care Mental health service.

There is also a comprehensive memory assessment service led by Dr. Juan Melendez (Associate Specialist)

(f) Drug and Alcohol Service

This service accept self-referrals and service referrals for people requiring support with drug or alcohol problems. The team consists a Service Manager, a Clinical lead Manager, a Consultant Psychiatrist (Dr Tanya Englebrecht), 3 Substance Misuse Practitioners, an Advanced Clinical Practitioner / Nurse Prescriber, a Harm Reduction Worker / Needle Exchange Worker, a Social Worker, an Arrest Referral Worker / Court Diversion Officer, a part time Substance Misuse Probation Officer, a Younger Persons Substance Misuse Worker, Alcohol Liaison Nurses, Community Detox Nurses, a Psychologist, a Counselling Student (supervised by Psychologist), a GP for 1 session per week and admin staff. They provide a range of treatments and interact closely with the third sector.

(g) Jersey Talking Therapies

Jersey talking therapies provides a range of psychological interventions in line with NICE guidance. Steps 2-3 are accessible by self-referral and Step 4 (PATS service) work more closely with JAMHS and take people through internal referral with more complex needs.

(h) Legislation team

This team is made up of the people who administer the mental health and capacity law in Jersey. It also contains the Authorised officers who undertake the mental health and capacity law assessments.

(i) Specialist Clinics and Pathways

JAMHS offers the following specialist clinics, each of which is supported by a consultant psychiatrist with a special interest in the area:-

· Perinatal Mental Health (Dr. Rachel Ruddy)

· Eating Disorders (Dr. Miguel Garcia)

· Prison In-reach (Dr. Bharat Irala)

· ADHD Clinic (Dr. Ulrich Muller-Sedgewick)

· Autism Clinic (Dr. Ulrich Muller-Sedgewick)

· Gender Identity Clinic (Dr. Zainab Kadim)

· CAMHS transition pathway (Dr. Max Marshall).

  • Recognised basic medical degree
  • Full GMC registration
  • Eligibility for inclusion on the Specialist Register or CCT in old age psychiatry (or within six months at time of interview) or equivalent
  • Higher speciality training or equivalent experience
  • Eligibility for Section 12 approval

Desirable

  • MRCPsych or equivalent approved by the Royal College of Psychiatrists.
  • Relevant higher degree, e.g. MD. PhD, MSc or other additional clinical qualifications
  • Section 12 approval
  • Sub-speciality certification in liaison psychiatry
  • Training or significant experience in early onset dementia or other speciality.
