Job summary

As a Community Registered Nurse within our District Nursing Team, youll deliver skilled, evidence based care to patients with complex needs in their homes, residential and other community settings. Whist working both independently and alongside a multi disciplinary team, youll assess, plan, and implement personalised care, that supports independence and helps prevent avoidable hospital admissions.
