Social Worker - Older Person's Community

Job summary

Join our supportive and dedicated Older Person's Community Team as a Social Worker for Manx Care on the beautiful Isle of Man! In this role, you will be primarily working with service users within the Older People Community Social Work Team to assess the care needs of service users aged 65+.

Main duties of the job

You will undertake Well-being Assessments to identify eligible need(s) and formulate individual care plans, promoting independence, autonomy and dignity and drawing upon strength-based approaches. Our service aims to maintain and enhance quality of life and wellbeing of older people by exploring family, friend, and local-community-based support systems to enable older people to remain at home. In addition to direct practice, you will also be involved in service development, consultation and other social work responsibilities, liaising with colleagues across the Public Service in Hospitals, Community, Residential and Day Care establishments, Domiciliary Services and Third Sector organisations.
