Job summary

Could you champion safer, smarter medication use across high-impact hospital services? As a Specialist Pharmacist - Polypharmacy and Acute Frailty you will take the lead in delivering high-quality clinical pharmacy services across the Frailty Unit and Acute and Emergency Medicine.

Main duties of the job

Your focus will be on ensuring the safe, effective, and appropriate use of medications whilst contributing to a Consultant lead Frailty Front Door service optimising patients for reduced admissions. You will also be responsible for supporting the Lead Clinical Pharmacist in shaping a proactive, responsive pharmacy service, providing advanced clinical care, and overseeing the training and development of Rotational Pharmacists and other multi-disciplinary team members. As part of our essential Pharmacy Department, youll work closely with other healthcare professionals to optimise patient care and ensure patients and their families receive the best outcomes from their medications.
