Looking to make a meaningful impact on dementia and delirium care in hospital? As a Dementia and Delirium Practitioner, youll be key in supporting patients in our Medical & Elderly Ward during their hospital stay, helping to create a safer, more supportive environment.
Main duties of the job
Youll help raise awareness of dementia and delirium across the organisation, though appropriate assessments and education, whilst establishing and supporting the delivery of diversional therapy, becoming a crucial link between the hospital, community and third sector organisations, and making sure people receive consistent care across every setting.
