Job summary

Ready to drive positive change in hospital social care? Join our Integrated Hospital Social Work Team as a Senior Social Work Practitioner and play a key role in improving outcomes for patients through safe, timely discharges and effective care planning.

Main duties of the job

As an experienced Social Worker, you will operate with a high degree of autonomy and authority using a range of evidence-based, social work interventions, manage complex cases, and support the professional development of the team around you. You'll collaborate closely with multidisciplinary teams, as well as our Complex Discharge and Access and Capacity Teams, to streamline transfer of care and discharge pathways and ensure patients with complex needs are supported to return home with the right care in place.
