Main duties of the job

To act as a Social Worker within the Adult Generic Team. The team provides an assessment of care needs and assists with the planning of safe discharges from hospital and adopting an integrated care approach within the Community. The social work service aims to support older adults with substantial disability, vulnerability or illness and those who appear to be an adult at risk.

About the role and our team

Join us as a Social Worker within our Adult Generic Team for Manx Care!

In this role, you will be providing high quality social work services to adults in our community from the age of 18 who may be considered at risk or in need of services. You will be responsible for the organisation and facilitation of assessments as well as the formulation and implementation of care plans for our users.

We undertake assessments and identify needs, often with people who dont necessarily fall within any other service. We work with a diverse spectrum across our community, includingworking closely with those who self-neglect and hoard, and support individuals to reach their goals.
