Job summary

The States of Guernsey are looking for a Palliative Care Specialist Nurse to join the Community Health and Social Care Service.

Main duties of the job

Adult Community Services is a multidisciplinary team with Community Nurses, Social Workers, Specialist Teams, Social Care and support staff on the same site. As a Palliative Care Specialist Nurse, the post holder will work collaboratively with multi-Professional teams in Primary Care, Oncology and the community including Hospice and site-specific teams in the Bailiwick of Guernsey to provide evidence-based palliative and supportive care for patients/clients and their families, prioritising people as individuals. The post involves acting as a resource for palliative and supportive care nursing provision, supporting clinical leadership, teaching and contributing to policy development.

To undertake the assessment of patients and clients with complex care needs including individuals with undifferentiated and undiagnosed life limiting conditions.

To plan, implement and evaluate specialised programmes of care utilising evidencebase to achieve optimum best clinical practice in palliative care and supportive care.

To provide specialist advice to staff in managing complex situations, ensuring that the patients/clients needs are met and that significant others are supported.

To independently liaise and communicate effectively with all members of the multidisciplinary team and other agencies involved in the care of the patient/client.

To initiate risk assessment processes to determine risks to the health and well-being of patients with complex care needs, taking relevant action to minimise these risks.

To promote patient/service user involvement in the planning, delivery and evaluation of care, respecting their wishes, beliefs and dignity.

To utilise information and data from a range of sources to identify patient/client issues and guide the decision-making processes.

To have an active role in education of HSC and outside agencies charities staff through in-service training, multi-disciplinary training programmes, student training and work experience.

To ensure that professional standards are upheld by the staff members working within the postholders sphere of responsibility.

To preserve safety by following and contributing to the development of policies, procedures & guidelines relating specifically to Palliative care nursing.

To be accountable for ensuring that safeguards are in place for patients/service users and escalate any safety, safeguarding or risk concerns following the appropriate processes

  • Registered Nurse
  • Current registration with NMC
  • Degree in related subject or demonstratable experience related to clinical area
  • Possess a full and valid driving licence and vehicle for business use is required for this
  • post; however reasonable adjustments will be made for individuals in line with the
  • Prevention of Discrimination (Guernsey) Ordinance, 2022

Desirable

  • Recognised Teaching qualification
  • Degree in related subject
  • Evidence of post-registration education relevant to clinical area
