Job summary

The States of Guernsey are looking for a Senior Mental Health Practitioner to join the Duty and Intervention Team based at the Oberlands Centre.The Duty and Intervention Team is the primary access point for specialist mental health services in Guernsey, providing timely assessment, brief evidence-based interventions, and signposting to appropriate care pathways. The team plays a critical role in managing referrals, conducting comprehensive mental health and risk assessments, and ensuring safe and effective triage for individuals presenting with a range of mental health needs.As a Band 6 Senior Mental Health Nurse Practitioner, you will deliver high-quality, person-centred care within a multidisciplinary team, working closely with service users, families, and partner agencies. Responsibility for Patient/Service User Care

1. To undertake the assessment of patients/service users with complex needs including individuals with unconfirmed diagnosis.

2. To develop, implement and evaluate specialised programmes of care utilising evidence and research.

3. To work autonomously, recognising changes in patients'/service users' presentation which require intervention, knowing when to escalate as appropriate.

4. To guide, advise and direct other staff in the assessment, planning, implementation and evaluation of specialised patient/service user care.

6. To utilise data and information from a range of sources to identify patient/service user condition to guide decision making.

10. To report all accidents and incidents as per policy and participate in risk management and Quality and Safety processes e.g SWARM.
