Job responsibilities
Clinical Governance
1. To be responsible for the provision and development of clinical pharmacy services to Medical, Surgical and all Wards in line with local and national Clinical Pharmacy Standards.
2. To have overall responsibility for Learning and Development programs within the pharmacy department coordinating and leading development sessions in conjunction with the Lead learning and development pharmacy technician, the associate director for pharmacy, the deputy chief pharmacist and the medicines management team.
3. To ensure appropriate training and supervision is in place to provide a safe, effective and sustainable pharmacy service for pharmacists and the whole medicines management team.
4. To have overall responsibility for Medicines Information. Coordinating and leading the departmental Medicines Information and Staff training program in conjunction with the Pharmacy Manager, Principal Clinical Pharmacist and the Medicines Management Team.
5. Investigate Clinical Incidents involving medications both within the Pharmacy dept. and throughout HSC, ensuring processes are in place to develop learning from these incidents.
6. To lead on the participation in the NHS Benchmarking project, and local KPI provision and monitoring.
7. To be responsible for the review of Patient Group Directives within the Bailiwick of Guernsey and the implementation and updating of protocols, guidelines, policies, within HSC which are medicines related.
8. To review the results of external audit and benchmarking related to Medicines Information and develop action plan as necessary.
9. To facilitate the undertaking and contribute to multi-disciplinary audit.
10. To be a member of and contribute to the Clinical Guidelines Committee.
11. Aid in the provision of evaluated drug expenditure data.
Management
1. To deputise for the Lead Pharmacist for Clinical Audit and Patient Safety in their absence in relation to day-to-day workload planning.
2. To participate in meetings and working groups relating to new Drugs and Policies relating to Medication usage and to work closely with the HSC Clinical Governance Team, ensure that policies relating to medicines and clinical services are in line with local and national directives.
3. To attend and advise HSC Governance Teams of any Safety Issues surrounding the provision and administration of Medicines within HSC and to report to Governance internal KPIs and external Benchmarking Information.
4. To provide advice to medical staff on medicines usage and expenditure.
5. To be a member of the Prescribing and Formulary committee as required.
6. To line manage early careers pharmacists / foundation trainee pharmacists as required.
Leadership
1. To act as the Pharmacy Lead for the provision of Medicines Information and Training.
2. To lead in the Clinical Pharmacy Support to in the areas of need.
3. To provide professional leadership to the Medicines Management team.
4. To be the secretary of the and cover for the Chair in their absence.
Service Development
1. To develop and manage the Learning and Development plan for pharmacists and the wider medicines management team.
2. To develop the Medicines Information Service for the Pharmacy service area and participate in the development of a Staff Training Program.
3. To be responsible for the development of a strategic plan for Training and Medicines Information.
4. Develop and monitor services to support an improved discharge service for patients, and carers ensuring a more effective transition from Secondary to Primary Care.
5. Initiate, plan, develop and implement clinical guidelines related to medicines use within HSC.
Education and Development
1. Plan and deliver training to pharmacy and other clinical staff on issues related to New Medicines or any issuesraised because of Clinical Audit. Supervising students on wards and acting as an educational supervisor as appropriate.
2. To provide training to the Medicines Management Team in the form of Clinical CPD meetings.
3. To provide training to ward and HSC staff, including the annual prescribing update to consultants employed at HSC.
4. The post holder will be committed to the concepts of continuing professional development and lifelong learning personally and for staff.
5. Contributing to the induction and competency-based training of the Clinical pharmacy team, Medicines Management Technicians and medical and nursing staff.
Clinical / Technical
1. To Participate in clinician ward rounds and multidisciplinary team meetings.
2. To provide a high level advanced clinical Pharmacy service to Medical, Surgical and Care all Wards. This includes medicines reconciliation, assessing prescription charts in order to ensure safe rational and effective prescribing, establishing drug histories, enabling use of patients own medicines and patient self-administration, and to encourage adherence to HSC formulary, local Whitelist and national guidelines.
3. Providing medications for individual patients during their stay in hospital ensuring adherence to Pharmacy policies and procedures relating to medicines management.
4. Provide professional supervision and in-process checking of dispensed medicines within the dept. whilst providing support to the Dispensary and Medicines
Management Team.
5. Clinically checking all prescriptions before dispensing and referring to the prescriber any unusual doses, potential drug interactions or incompatibilities for confirmation or revision before authorising dispensing.
6. Working as part of the Medicines Management team ensuring the provision of a responsive, high-quality, safe and effective service assisting in the day-to-day management of staff and workload.
7. Provide information and advice on the safe, effective and economical use of drugs to health professionals and patients. Visiting allocated wards on a daily basis, prioritising activity as appropriate.
8. Discussing with patients any issues about their medication and liaising with other health care professionals in Primary and Secondary Care as appropriate.
9. Assessing the appropriateness and safety of prescriptions in terms of dosing, biochemistry, disease management and allergy and recommending changes to treatment where appropriate to improve effectiveness of drug therapy and patient outcomes.
10. To transcribe Drug Charts on wards for patients whose stay is longer than the drug charts validity.
11. To be / to become a Pharmacist Independent Prescriber (IP) to further improve medicines management at clinic or ward level.
12. To cover the lead Pharmacist Clinical Audit and Patient Safety in their absence.
13. To support the Principal Clinical Pharmacist in the provision of and Admissions and Discharges Pharmacy Service.
Financial Responsibilities
1. To promote high-quality, cost-effective prescribing.
2. To assist with horizon scanning and assessment of cost implications of changes to the use of medicines in the clinical areas in which the post holder is working.
3. To provide information to the associate director of pharmacy and the high-cost drugs pharmacist in relation to any high-cost drug spend including identifying causes and potential impact of over-spend.
4. To work in conjunction with the Clinical Audit and Patient Safety Pharmacist to ensure that due process is followed when a new drug request is made, ensuring that cost effectiveness of new drugs is proven and documented.
5. To support your manager to make efficient and effective use of resources. All staff are responsible for identifying any actual or potential deviation from budgets and are to work with the budget holder or manager to find effective ways of handling it.
6. To use resources in a manner consistent with organisational objectives and policies, ensuring that resources are realistic, justified and of clear benefit to the organisation.
Administrative
1. To ensure that all pharmacy paperwork and documentation is completed clearly and accurately and filed and archived appropriately.
2. To ensure that all Patient Records are maintained within the bounds of Professionalstandards, and Data Protection Laws.
3. To ensure that all monitoring and audit data is recorded and reported upon in a timelyfashion and audit trails are maintained and accessible.
4. To ensure that all clinical incidents are reported and acted upon in a time frame as laidout in HSC Guidelines.
5. To ensure that all drug chart endorsements and transcriptions are done in a neat and readable manner.
General Duties
1. The provision of a high-quality service, delivered in professional and courteousmanner.
2. Promoting positive and effective communication within the Pharmacy Department.
3. To act as the Responsible Pharmacist in accordance with General PharmaceuticalCouncil (GPhC), legislation and departmental SOPs.
4. Establish links with other pharmacists to share / learn from examples of best practice
5. To adhere to and ensure that all staff follow HSC and departmental Health and Safety,COSHH guidelines and departmental procedures.
6. Undertake any other training and duties as required by HSC or the associate directorof pharmacy.
7. To participate in the Saturday, pharmacy bank holiday, flexible working weekend andevening Rotas, as required to meet the needs of the service.
8. To participate in the on-call rota.
9. Receive, store and supply Controlled Drugs in accordance with the Misuse of DrugsAct.
10. Contribute to the effectiveness of the pharmaceutical service by putting forwardsuggestions for innovation and development in professional, technical and managerialaspects of the work.
11. To participate and contribute to Patient Safety Meetings and where appropriateattend Clinical Risk Meetings.
12. To practice at all times in accordance with the General Pharmaceutical Council (GPhC)code of ethics.
