Job responsibilities
CLINICAL
Work closely with other specialities within the multidisciplinary team to provide a comprehensive individualised out-patient cardiac rehabilitation service to deliver the core components of cardiac rehabilitation as outlined by the BACPR.
Lead and participate in the provision of early and core cardiac rehabilitation using a variety of modes of delivery to meet the needs of patients and deliver the core components of cardiac rehabilitation. Modes of delivery include group-based programmes, home programmes, the Heart manual, web-based programmes or a combination of these, i.e. a hybrid programme.
Undertake initial and ongoing assessment of people with cardiovascular disease using appropriate assessment methods to develop appropriate care packages and set SMART goals.
Provision of advice on healthy lifestyle behaviours, risk factor reduction, smoking cessation, management of condition, stress management and dietary advice.
Demonstrate expert knowledge in cardiology and cardiac rehabilitation and act as specialist clinical advisor to Health and Social Care, its employees and consumers.
Communicate effectively with clients, carers and families providing relevant support, information, advice and counselling.
Act as the clients advocate when necessary, especially in decision making to enable clients to make informed choices.
Develop initiatives to ensure clients views of the service are sought and they are actively involved in the development, monitoring and evaluation of the services they receive.
Develop clinical guidelines, comprehensive protocols and care pathways for the management and secondary prevention of cardiovascular disease (cardiac rehabilitation) reflecting the agreed milestones of both the National Service frameworks and local standards.
Develop and initiate new clinical practice in all aspects of cardiac rehabilitation, working in a collaborative manner with colleagues and members of the multi-disciplinary team.
Recognise and promote the importance of accurate documentation in assessment, treatment and discharge information for each patient, and to take action to rectify where inadequacies are found.
Be responsible for accurate data input and safe management of the National Audit for Cardiac Rehabilitation Database.
EDUCATION AND DEVELOPMENT
Create a learning environment, which meets the needs of the multi-disciplinary team providing cardiac rehabilitation services.
Maintain a personal professional profile, identifying own areas for professional development and negotiating how best these needs can be met.
Continually update and develop practices within the scope of professional practice.
Develop, deliver and evaluate clinical training and education support for health care professionals working with clients who have cardiovascular disease.
Act as a resource within specialist field within wider community, i.e. nursing, residential homes and primary care settings.
Work in collaboration with educationalist links. Develop teaching packages for use as educational tools.
Promote the education of the patients and relatives and develop literature, aids and information to meet the needs of the patient.
Apply appropriate learning theories to client education including health promotion and illness prevention strategies.
Develop networks with colleagues in the UK and abroad to share information and ensure best practice.
SERVICE LEAD
Assess, plan, implement and evaluate individual programmes of care with accurate documentation, delegating effectively to others as required.
Act as a role model and lead in an innovative and imaginative manner, challenging in a constructive manner by the use of expert knowledge and advanced levels of understanding and synthesis of information.
Define, influence and implement departmental objectives, which reflect organisational goals through active participation in local and national meetings.
Take a lead in the review of the cardiac rehabilitation service locally and act as a change agent in implementing the recommendations of the review of the service.
Lead the team towards achieving common objectives expressing a clear idea of direction.
Manage change effectively within the cardiac rehabilitation service to improve practice and health outcomes.
Actively participate in the recruitment and selection of staff
Monitor staff performance and identify individual development needs.
Facilitate, develop and monitor quality initiatives.
Ensure that Health and Social and States of Guernsey policies are adhered to.
Promote collaborative working relationships and effective communication between all members of the multi-disciplinary team, both internally and externally.
Maintain effective information systems at a department level and act as a highly effective communicator.
Ensure written paper and electronic communication complies with organisational and professional standards.
RESEARCH
Promote research-based practice and clinical effectiveness by actively and regularly updating ones own and others professional knowledge through journals, information technology and publications.
Continually evaluate practice in the department developing plans for change where appropriate and acting as change agent.
Promote and disseminate research findings.
In conjunction with Consultant Physicians for Cardiology, participate in research and audit.
Promote the excellence of Health and Social Care by producing papers on work accomplished in the cardiac rehabilitation arena for publication.
Participate in the annual National Audit of Cardiac Rehabilitation and other audits to justify own practice and clinical decision-making skills.
In conjunction with the Consultant Physician for Cardiology, advise and monitor service provision providing epidemiological data within the Bailiwick of Guernsey.
Ensure that clinical practice, policies, procedures and standards of care are evidence based.

Comply with the principles contained in the Data Protection (Bailiwick of Guernsey) Law 2017 concerning improper disclosure, misuse or breach of confidentiality, in respect of information held on computer system or otherwise.
