Main duties of the job

The post holder will work as part of a multidisciplinary primary healthcare team delivering primary healthcare nursing including elements of occupational health within the Kent Combined Medical Practice. This will include working with the Senior Practice Nurse or Senior Medical Officer within the medical facility. The combined Practice is formed of 3 sites - Maidstone, Shorncliffe and Chatham. There is an expectation that you will work across all 3 sites when required.

Job responsibilities

Duties and responsibilities for the role include but are not limited to:

  • Supporting with the care of patients with chronic illnesses e.g. Asthma, Diabetes, Coronary heart disease (CHD) and hypertension ensuring continued maintenance of the Chronic Disease Register.
  • Assisting with health and well-being clinics (e.g. smoking cessation, weight management, alcohol misuse) using health promotion, health education, screening and therapeutic communication skills.
  • Providing advice and support to patients, their families and other healthcare professionals following assessment, diagnosis and through treatment.
