Job summary

We are looking for a dynamic, forward thinking and dedicated senior professional to join our highly specialised team of CHC practitioners. The successful candidate will be responsible for providing clinical expertise to the outcome of CHC and business processes for the Department or Directorate.

Main duties of the job

  • Will be responsible for the direct delivery of the Continuing Healthcare service, developing and maintaining the highest levels of relationship management with customers and other stakeholders
  • To provide extensive clinical leadership and operational management for the clinical pathway for AACC, including in the areas of clinical governance and clinical safety
  • To provide clinical expertise to the team and other stakeholders. For example, complex case management and Court of Protection applications
  • To ensure that allocated resources are effectively and efficiently managed.
  • To ensure the timely, effective, high-quality delivery of key service developments and KPI's

You will be joining the team at an important time and will have a big contribution to make towards improving assurance around the quality of the Clinical Service provided by the team. Working closely with administration/placement and financial colleagues and the broader team you will be able to effectively plan and manage the change required to improve the quality and consistency of the clinical service delivery.

Job responsibilities

To provide extensive clinical leadership and operational management for the clinical pathway for AACC, including in the areas of clinical governance and clinical safety.

To ensure the timely, effective, high-quality delivery of key service developments.

The post holder will be personally responsible for the direct delivery of the Continuing Healthcare service, developing and maintaining the highest levels of relationship management with customers which would include ICBs, System Partners, Local Authorities, and local NHS England /Local Area Team members.

The post holder will have demonstrable proven track record at a senior management level of successfully securing customer confidence and business through effective marketing of services and delivery of strategic and operational priorities.

The post holder will also demonstrate a track record of managing a large complex team and a robust knowledge of the NHS framework for Continuing healthcare, the legal framework around Mental Capacity Act, PHBs and the CHC interface with Section 117
