All Age Continuing Healthcare is looking for an All Age Continuing Care Assessor Apprentice to join the team.

The primary aim of the Assessor role is to liaise with key stakeholders, service users and their families to determine eligibility for NHS continuing healthcare and funded Nursing Care in line with the National Service Framework. The implementation of a care package and review of service in line with the National Framework.

This post will support the Head of Department and their team to ensure that the Continuing Healthcare Care workstream is planned and managed effectively to support successful delivery. They will assist the Senior Nurses ensuring correct processes are followed in delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised July 2022)

Main duties of the job

As a CHC Assessor you will support the screening and assessment of individuals applying for and already in receipt of CHC/FNC packages of care. You will be responsible for co-ordinating the most vulnerable people in society to ensure they are in receipt of the correct care.

You will have the opportunity to liaise and work closely with Social Services, providers, patients and their families and other relevant agencies in assessing and supporting individuals, as well as developing care plans and review processes and making changes to care packages in accordance with the needs of the individual.

You will work within a strong, friendly, and supportive team, you will be allocated a mentor and line manager. Key responsibilities of this role will include conducting assessments, case co-ordination, decision making and applying the MCA.
