Job summary
This is an exciting opportunity to work with South Sefton Primary Care Network, our GP Federation, South Sefton Primary Health Care Limited and our 19 constituent practices.
The post holder will be a caring, compassionate and committed newly qualified GP who will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the PCN team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required.
This service will work as part of the overall Enhanced Access Service. Some evening and weekend work will be required in line with the requirements of the PCN Enhanced Access Service specification.
The NHSE requirements of this scheme mandate that applicants must have qualified on or after 1 June 2023 and must not have had substantive employment within a GP practice.
There is complete flexibility about both the number of sessions and days successful postholders can choose to work
Main duties of the job
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with and adherence to prescribed treatmentsProvide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation

South Sefton Primary Care Network is a large PCN covering the four localities of South Sefton, with a population of just over 155,000 patients.
PCNs are groups of GP practices working with a wide range of partners across the health and care system to improve the health of their population. Key to this purpose is strengthening general practice provision through working together, where it makes sense to do so and working as part of integrated care teams which work collaboratively for and with local people in each locality.
