Job summary

The post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within a Primary Care Network (PCN) and general practice. The post holder will contribute to providing nursing care to patients registered with a GP practice within the South Sefton PCN. Working under the supervision of a Registered Nurse the post holder will focus on operating across specified areas, e.g. learning disability annual review and follow ups, Severe mental illness (SMI) health checks, vocational and employment advice to patients, activities coordination, assisting in individual and therapeutic group work. Work with registered nurses to deliver individual treatment packages with an emphasis on collaborative care that focuses on wellbeing and positive planning and outcomes. The post holder will work with other members of the Multidisciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patient.

Main duties of the job

Carry out LD annual health checks, ongoing reviews and follow ups

Carry out SMI health checks, ongoing reviews and follow ups

Carry out long term condition reviews and any necessary follow ups

Work with registered nurses delivering on key priorities areas across the PCN.

Delivering high quality, compassionate care

Promoting health and independence for patients

Delivering holistic care with a whole person approach

Contributing to MDT and onward discussions

Working across a number of GP practices within the South Sefton PCN area

Nurse Associate Qualification is essential. You will need this otherwise you will not be able to fulfill the role.
