Job summary

Exciting opportunities are now available to join the Continuing Healthcare team (CHC) in the role of PHB / Care Management Co-ordinator for 30 hours a week.

We are looking for candidates from a diverse range of backgrounds that can evidence how their qualifications, skills and experience will contribute to NECS values and strategic goals. Main duties of the job

This role will be responsible for:

Undertaking care management duties, in order to meet the assessed needs of patients who receive NHS Continuing Care funding, communicating and working in partnership with GPs, social services and other statutory and voluntary organisations including patients and their carers.

Promoting Personal Health Budgets (PHBs) ensuring integration into continuing healthcare and that they are offered where requested to those clients who meet agreed criteria.

Case managing CHC clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective.

To identify the risks presented by the needs of individuals and develop person centred, cost effective care plans, working with specialist health professionals and the patient to manage these risks.

This post will support the on-going development of the Personal Health Budget (PHB) for Tees CCGs. This will include assisting with the introduction of Direct Payments for Health Care and the overall care management of clients.

The key principles of PHB and care management is:

To allow individuals to have more choice and control over their health and well-being needs.

To facilitate a cultural change in both individuals and professionals towards how outcomes can be achieved.

To stimulate flexibility and creativity towards the personalisation of service.

To use the experiences of personal health budget users to better inform and shape future commissioning intentions.

To assist with World Class Commissioning by stimulating the market to ensure the demand for services is met.

To seize the opportunity as one of the leading organisations nationally to be at the forefront of innovation and transformation.

To help patients identify and commission resources that enable them to choose the best support options that fit their assessed need and preferred lifestyle choices.

Provide on-going assistance, where required, in supporting patients, in the management of their support. The post holder will work directly with the patient and/or their family, making sure they stay in control and live the life they choose.

Qualifications

Essential

  • Professional qualification in Social Work or Registered Nurse qualification.
  • Registered with the HCPC or the NMC.
  • 2 years post qualifying experience.

Experience

  • Working with adults in a social care or health care setting.
  • Experience of working in partnership with service users, carers, service providers and other professionals.
  • Experience of Safeguarding adults.

Desirable

  • Management experience in the NHS/Social Care.
